区域阻滞麻醉在老年腹股沟疝无张力修补术的临床应用

Application of tension-free hernia repair under field block in the elderly patients with inguinal hernia

目的 观察区域阻滞麻醉在老年患者无张力疝修补术中的临床效果。方法 回顾性分析2013年3月～2014年5月收治的80例老年腹股沟疝患者,按照麻醉方式分成腰-硬联合麻醉组（A组）,区域神经阻滞麻醉组（B组）,观察记录手术时间、麻醉效果、术中生命体征变化、镇痛时间及并发症发生情况。结果 B组麻醉前后血流动力学稳定,A组在T1时收缩压明显降低（P〈0.05）,A组发生尿潴留21例,腰背痛5例,与B组比较差异有统计学意义（P〈0.05）。结论 区域阻滞麻醉操作简单、效果良好、安全性高,特别适用于高龄危重患者。

Objective To explore the value of tension-free hernia repair under field block in the elderly patients with inguinal hernia. Methods From March 2013 to May 2014,80 patients undergoing unilateral inguinal hernia were ramdonly divided into CSEA（group A） and field block（group B）.The operation time, anesthesia effect.The basic vital signs during operation.postoperative pain and complications were recorded. Results Group B had can steady hemodynamic during before anesthesia and after anesthesia,group A at T1 had lower blood pressure（P 〈 0.05）.Compare with group B there were 21 cases of urinary retention,5 cases low back pain in group A（P 〈 0.05）. Conclusion Field block with a definite effect is easy to handle,high safety and less complication,especially for high risk eldery patients with inguinal hernia.
