两种不同手术治疗慢性肛裂的有效性及安全性研究

Effectiveness and safety study of two different surgical methods in the treatment of chronic anal fissure

目的 比较两种不同手术治疗慢性肛裂的有效性及安全性。方法 选取2013年4月-2014年6月本院收治的60例慢性肛裂患者作为研究对象,随机分成实验组和对照组,各30例。实验组采用后位内括约肌节段式部分切开术进行治疗,对照组采取后位内括约肌切开术进行治疗,比较两组的治疗效果。结果 两组的治疗总有效率比较,差异无统计学意义（P〉0.05）。实验组的手术操作时间、术后愈合时间显著短于对照组,差异有统计学意义（P〈0.05）。实验组的术中出血量显著少于对照组,差异有统计学意义（P〈0.05）。实验组的并发症发生率显著低于对照组,差异有统计学意义（P〈0.05）。结论 内括约肌节段式部分切开术具有操作简单、安全、创伤小、并发症少、术后愈合快等优点,值得临床推广应用。

Objective To compare the effectiveness and safety of two different surgical methods in the treatment of chronic anal fissure. Methods 60 patients with chronic anal fissure in our hospital from April 2013 to June 2014 were selected and randomly divided into the experimental group and the control group,30 cases in each group.The experimental group was treated with the segmental partial posterior internal sphincterotomy,the control group was treated with posterior internal sphincterotomy.The therapeutic effect of two groups was compared. Results There was no significant difference of the total therapeutic effective rate in two groups（P〉0.05）.The operation time and postoperative healing time of the experimental group was shorter than that of the control group,with significant difference（P〈0.05）.The intraoperative amount of bleeding of the experimental group was less than that of the control group,with significant difference（P〈0.05）.The incidence rate of complication in the experimental group was lower than that in the control group,with significant difference（P〈0.05）. Conclusion The segmental partial posterior internal sphincterotomy has simple operation,safe,less trauma,less complication,postoperative healing fast and other clinical advantages,it is worthy of clinical promotion and application.
