高血压合并主动脉夹层52例临床诊治分析

Hypertension combined Clinical Analysis on Diagnosis and Treatment of 52 Cases with Aortic Dissection

目的:探讨高血压合并主动脉夹层的临床治疗方法。方法:选择本院2008年12月-2012年12月收治的104例高血压合并主动脉夹层患者作为研究对象,并按随机数字表法分为试验组和对照组,每组各52例。对照组患者单纯使用药物治疗方法,试验组患者联合药物治疗方法和外科手术方法,对比观察两组患者的治疗效果。结果:治疗后,试验组患者平均舒张压为(76.5±8.6)mm Hg,平均收缩压为(101.4±5.7)mm Hg,而对照组患者分别为(85.7±6.9)mm Hg、(115.7±4.8)mm Hg;试验组患者血压控制有效率为80.77%,对照组为69.23%;试验组患者主动脉夹层治疗总有效率为90.38%,对照组为76.92%,试验组疗效明显优于对照组,比较差异具有统计学意义(P<0.05)。结论:相对于单纯使用药物的治疗方法,在药物治疗基础上联合使用外科手术治疗高血压合并主动脉夹层具有更好的治疗效果。因此,该方法值得在临床上进一步推广和应用。

Objective： To study hypertension combined the clinical treatment of aortic dissection. Method： In our hospital from December 2008 to December 2012, 104 hypertension patients combined with aortic dissection were selected as the research objects, and were randomly divided into experimental group and control group, 52 cases in each group. The control group received simple use of drug therapy for treatment, and the experimental group was given combined drug therapy and surgical methods. Therapeutic effect between the two groups was compared. Result： After treatment, average diastolic blood pressure and average systolic blood pressure of the experimental group was （ 76.5 ~ 8.6 ） mm Hg and （ 101.4 ~ 5.7 ）mm Hg, and those of the control group was （ 85.7 ~ 6.9 ） mm Hg, and （ 115.7 ~ 4.8 ） mm Hg, respectively. Blood pressure control effective rate and total effective rate of aortic dissection treatment of the experimental group was 80.77% and 90.38%, and those of the control group was 69.23% and 76.92%, It showed that experimental treatment effect was significantly better than that of the control group, the differences were statistically significant （ P〈O,05 ） . Conclusion： Compared to drug treatment, on the basis of drug treatment combined surgical treatment of high blood pressure with aortic dissection has better therapeutic effect. Therefore, this method is worthy of further promotion and application in clinic.
