生长抑素联合泛影葡胺治疗术后早期炎性肠梗阻的临床效果

Clinical effects of somatostatin combined with meglumine diatrizoate in the treatment of early inflammatory intestinal obstruction

目的 探讨生长抑素联合泛影葡胺治疗术后早期炎性肠梗阻的效果。方法 选择本院2008年5月~2014年12月收治的58例术后早期炎性肠梗阻患者分为两组,各29例。A组给予常规治疗,包括绝对禁食、抑制胃液分泌、持续胃肠减压、全胃肠外营养支持、应用抗生素防治感染等措施。B组在采取常规治疗方法基础上加用生长抑素6 mg应用微量泵经外周静脉持续24 h均匀泵入,同时将76%复方泛影葡胺100 ml经胃肠减压管注入,2次/d,在注入药物后夹闭胃管3 h,治疗过程中密切观察患者用药后腹部情况。结果 B组胃管引流液量、肛门通气或通便时间、腹痛减轻时间、中转开腹率、平均住院日优于A组,差异有统计学意义（P〈0.05）。结论 生长抑素联合泛影葡胺能明显缓解术后早期炎性肠梗阻患者的临床症状,提高该类患者保守治疗的成功率。

Objective To explore the curative effects of somatostatin combined with meglumine diatrizoate in the treatment of postoperative early inflammatory intestinal obstruction. Methods 58 patients with postoperative early inflammatory intestinal obstruction from May 2008 to December 2014 in our hospital were assigned to group A and group B,with29 cases in each group.Group A was given regular treatment,including absolute fasting,inhibition of secretion of gastric juice,continuous gastrointestinal decompression,total parenteral nutritional support,application of antibiotics for the prevention of injection and other measures.Group B was further given even pumping of 6 mg somatostatin for continuous24 h by micropump via peripheral vein on the basis of the regular treatment,and at the same time 100 ml 76% of complex meglumine diatrizoate was injected via gastrointestinal decompression,2 times/day.After the injection of drugs,the gastric tube was clipped for 3 h.The abdominal conditions after the use of drugs were closely observed during the course of treatment. Results Compared with group A,drainage volume by gastric tube,time of anal passage of gas or movement of bowels,time of alleviation of abdominal pain,conversion rate to laparotomy and average length of stay in group B all showed significant differences（P〈0.05）. Conclusion Somatostatin combined with meglumine diatrizoate is able to significantly alleviate the clinical symptoms of patients with postoperative early inflammatory intestinal obstruction,and improve the successful rate of the conservative treatment for such kind of patients.
