社区护理干预在脑卒中合并高血压患者中的应用分析

Application analysis of community nursing intervention in patients with cerebral apoplexy and hypertension

目的 探讨社区护理干预在脑卒中合并高血压患者中的应用效果。方法 选取2013年10月~2014年9月本社区的108例脑卒中合并高血压患者,将其随机分为观察组和对照组,每组54例。对照组接受一般的家庭护理,观察组在家庭护理的基础上接受社区护理。比较两组患者的血压水平、对医嘱的依从性、复发率及并发症发生率。结果 干预前,两组的血压水平差异无统计学意义（P〉0.05）;干预后,观察组的血压为（76.9±7.8）/（121.2±6.9）mm Hg,对照组为（88.7±9.2）/（134.7±9.5）mm Hg,两组差异有统计学意义（P〈0.05）。观察组的医嘱依从性评分为（14.37±1.4）分,显著高于对照组的（9.55±2.7）分,两组差异有统计学意义（P〈0.05）。干预后,观察组的并发症发生率和复发率低于对照组,差异有统计学意义（P〈0.05）。结论 对脑卒中合并高血压患者实施社区护理干预,可有效保证患者的医嘱依从性和良好的生活习惯,督促患者控制血压水平。

Objective To evaluate the application effect of community nursing intervention in patients with cerebral apoplexy and hypertension. Methods 108 patients with cerebral apoplexy and hypertension from October 2013 to September 2014 were selected and randomly divided into observation group and control group.Each had 54 cases.In the control group,general home care was applied,while in the observation group,on the basis of the home care,community care was provided.The blood pressure level,compliance of doctor′ s advice,recurrence rate and incidence of complications after intervention in the two groups were compared. Results Before intervention,the blood pressure was no significant difference between two groups（P 0.05）.After intervention,the blood pressure level in the observation group was（76.9±7.8）/（121.2±6.9） mm Hg,while in the control group was（88.7 ±9.2）/（134.7±9.5） mm Hg,the difference was significant（P〈0.05）.The score of compliance of doctor′s advice in the observation group was（14.37 ±1.4） points,significantly higher than that in the control group[（9.55±2.7） points],the difference was significant（P〉0.05）.After intervention, the incidence of complications and recurrence rate in the observation group were lower than those in the control group,the differences were significant（P〈0.05）. Conclusion Application of community nursing intervention in patients with cerebral apoplexy and hypertension effectively ensures patient′s compliance of doctor′s advice and good habits of life,and control patient′s blood pressure levels.
