摘要
目的:探讨血浆D-二聚体水平在食管癌患者诊治中的临床价值。方法:测定120例食管癌患者血浆D-二聚体水平,并与健康对照组进行比较,分析食管癌患者术前D-二聚体水平与临床病理特征的关系及手术前后D-二聚体水平变化。结果:食管癌组术前血浆D-二聚体水平显著高于健康对照组,并随TNM分期的进展而升高,有并发症者术前D二聚体水平显著高于无并发症组,术后2周D-二聚体水平显著低于术前,Ⅲ~Ⅳ期患者下降更为显著,有并发症组术后2周D-二聚体水平仍显著高于健康对照组(P〈0.05)。结论:D-二聚体检测方法灵敏度高,可作为食管癌患者辅助诊断、治疗及手术风险评价的指标。
Objective: To explore the value of D-Dimer level in the diagnosis and treatment for patients with esophageal carcinoma. Method: The level of D-Dimer of 120 eases of patients with esophageal carcinoma was tested before and after operation, and was compared with healthy eobert.Exploring the relationship between the level of D-Dimer and the clinical and patholagieal parameters, analyzing the variation of D-Direct before and after esophagectomy.Result: The D-Dimer level of patients with esophageal cancer was higher than that of the healthy cohort.Compared with early stage, the level of D-Dimer was higher in patients with advanced TNM stage cancer.Groups were comparable in clinical features.The increase of preoperative D-Dimer was related to postoperative combination.The D-Dimer of the most patients decreased dramatically 2 weeks after the treatment of surgery, especially in the Ⅲ-Ⅳ TNM stage cases, however the level of the patients with serious postoperative combinations was still higher than the healthy eohort(P〈0.05).Conclusion: D-Dimer is a reliable sensitive factor and is helpful for the auxiliary diagnosis and treatment.It is also a predictor of the surgical risk for the esophageal carcinoma.
