青光眼合并白内障三联手术临床分析

Clinical analysis of triple combination operation on the patients of coexisting glaucoma and cataract

目的 观察分析超声乳化白内障吸除、人工晶状体植入配合小梁切除术治疗青光眼合并白内障的临床疗效和安全性.方法 选取2012年2月至2014年2月的124例青光眼合并白内障患者为研究对象,根据治疗方法不同将患者分为观察组（69例）和对照组（55例）,两组均采用透明角膜缘切口超声乳化人工晶状体植入术治疗,观察组在此基础上联合小梁切除术进行治疗,比较两组患者的临床治疗效果以及术后不良反应发生率.结果 观察组治疗后眼压改善总有效率（96.65％）显著高于对照组（81.82％）,两组比较差异有统计学意义（x2=62411,P ＜0.05）.观察组视力改善总有效率（100.00％）明显高于对照组（90.91％）,两组比较差异有统计学意义（x2 =6.5363,P＜0.05）.观察组术后不良反应发生率（18.18％）显著低于对照组（4.35％）,两组比较差异有统计学意义（x2 =6.2411,P＜0.05）.结论 超声乳化白内障吸除、人工晶状体植入配合小梁切除术治疗青光眼合并白内障不仅可以提高患者的治疗效果,而且术后不良反应发生较少,无需二次手术,显著减轻了患者的痛苦,值得在临床上大力推广.

Objective To observe the effects of triple combination operation on the patients of coexisting glaucoma and cataract.Methods One hundred and twenty-four cases of glaucoma and cataract from February 2012 to February 2014 were divided into two groups according to the type of their previous treatment：the control group（55 cases） were given clear corneal incision phacoemulsification combined with intraocular lens implantation,while the observation group （69 cases） were given the clear corneal incision phacoemulsification and intraocular lens implantation combined with trabeculectomy.Results The total effective rate（reduce intraocular pressure and improve of visual acuity） were 96.65% and 100.00% in the observation group,which were higher than those in the control group （81.82% and 90.91%）;besides,the incidence rate of adverse reaction in the observation group（18.18%） was significant lower than that in the control group（4.35%）.Conclusions The triple combination operation for the patients of coexisting glaucoma and cataract is effective and safe with less complication,and no need to have a second operation,so it is worthy of clinical promotion.
