摘要
目的:探讨超声、MRI、CT诊断脂肪肝的优势及劣势。方法:选取我院行肝脏穿刺活检或部分肝切除证实的脂肪肝患者40例,给予超声、CT及MRI检查,观察比较超声、CT、MRI图像特点,以多变量指标定量或半定量方法对肝脏脂肪含量(LFC)进行测定,同时将病理检查作为对照,进行综合比较。结果:本次研究中,40例脂肪肝患者CT检查的诊断准确率为95.0%,超声检查准确率90.0%,MRI检查准确率75.0%,超声与CT检查准确率差异无统计学意义(P>0.05);CT和超声准确率均高于MRI,差异具有统计学差异(P<0.05)。结论:脂肪肝的影像学检查主要是超声和CT,而MRI的检出率较低;脂肪肝B超检查的诊断率虽然低于CT检查,但其敏感性较高,操作便捷无放射性损伤,临床检测费用较低,推广使用价值大。
Objective: To discuss advantages and disadvantages of ultrasound,MRI,CT in diagnosis of fatty liver. Methods: 40 fatty liver patients in our hospital confirmed by a liver biopsy or partial liver resection were given ultrasound,CT,MRI examination,the characteristics of ultrasonic,CT,MRI image were observed and compared,liver fat content( LFC) was measured by multivariate index quantitative or semi quantitative method,at the same time. The pathological examination was as control,and comprehensive comparison was done. Results: In this study,40 cases of patients with fatty liver whose diagnosis accurate rates of CT examination were 95. 0%,ultrasound examination accuracy rates were 90. 0%,the accuracy rates of MRI examination were 75. 0%. Data analysis showed,in comparison of accuracy rates,there was no differences between ultrasound and CT examination( P〉0. 05); CT and ultrasound accuracy rate was higher than that of MRI,the difference was statistical significance( P〈0. 05). Conclusion: Imaging examination of fatty liver is mainly ultrasound and CT,while the detection rate of MRI is lower. Diagnosis rate of fatty liver B ultrasound examination is lower than that of CT examination,but it has higher sensitivity,handling is convenient and no radiation damage,clinical detection cost is lower,and thus it is worthy of popularizing.
