摘要
目的:探讨肠内肠外营养支持联合微生态制剂治疗消化道瘘的临床疗效。方法:选择消化道瘘患者65例,按随机数字表法分为观察组(n=33)和对照组(n=32)。对照组采用常规肠内肠外营养支持治疗,观察组在对照组基础上增加微生态制剂治疗,观察两组患者治疗前、治疗后10 d、30 d时的体重、血清白蛋白、前白蛋白、转铁蛋白的改变情况及消化道症状发生情况。结果:两组治疗后10 d、30 d患者的体重、血清白蛋白、前白蛋白、转铁蛋白较同组治疗前升高(P<0.05);治疗30 d时,观察组体重、血清白蛋白、前白蛋白、转铁蛋白明显高于对照组(P<0.05)。观察组消化道症状发生率为21.2%,明显低于对照组(46.9%)(P<0.05)。结论:肠内肠外营养支持联合微生态制剂治疗消化道瘘可有效改善患者的营养状况,增加体重,纠正微生态失衡,减少胃肠道症状,促进康复。
Objective: To investigate the clinical effect of treating digestive tract fistula with enteral and parenteral nutrition combined with probiotics preparation. Methods: 65 patients with digestive tract fistula were selected,according to digital table method,they were randomly divided into observation group( n = 33) and control group( n = 32). control group were treated with conventional enteral and parenteral nutrition support,observation group were added probiotics preparation on basis of control group. The changes of body weight, serum albumin,prealbumin,transferrin before treatment,at tenth day after treatment,at thirtieth day after treatment between two group,as well as theincidence of gastrointestinal symptoms were oberved. Results: The body weight,serum albumin,prealbumin,transferrin at tenth day after treatment,at thirtieth day after treatment in two group gradually increased than those before treatment in the same group( P〈0. 05). at thirtieth day after treatment,the body weight,serum albumin,prealbumin,transferrin in observation group were obviously higher than those in control group( P〈0. 05). The incidence of gastrointestinal symptoms in observation group was 21. 2%,which was obviously lower than that in control group( 46. 9%)( P〈0. 05). Conclusion: Ttreating digestive tract fistula with enteral and parenteral nutrition combined with probiotics preparation can effectively improve nutritional status of patients,increase weight and correct micro ecological imbalance,reduce gastrointestinal symptoms,promote rehabilitation.
