不同炎症程度下颌第三磨牙拔除术后并发症对比研究

Comparative study of postoperative complications of mandibular third molar extraction under different degree of inflammation

目的:对比不同炎症程度下颌第三磨牙拔除术后并发症的发生率,为牙拔出时机选择提供理论依据。方法:将180例下颌第三磨牙拔除患者分为无炎症组、炎症组、炎症缓解组,每组60例,拔除后记录术后拔牙创出血、面部肿胀、局部疼痛、张口度、干槽症发生率及痊愈疗程,并进行统计学分析。结果:炎症组与炎症缓解组和无炎症组相比,术后拔牙创出血、面部肿胀、局部疼痛、张口度及痊愈疗程,差异均有统计学意义(P<0.05),但干槽症发生率差异无统计学意义(P>0.05)。结论:选择无炎症期与炎症缓解期牙拔出可改善术后拔牙创出血、面部肿胀、局部疼痛及张口度,缩短痊愈疗程,但炎症期牙拔除并不会增加干槽症发生率。

Objective： To compare the incidence of postoperative complications of mandibular third molar extraction under different degree of inflammation and provide a theoretical basis for the timing of tooth pulled out.Methods： 180 cases of mandibular third molar extraction patients were divided into group without inflammation,inflammation group,relieve inflammation group,60 cases in each group. Bleeding after tooth extraction,facial swelling,local pain,mouth opening,the incidence of dry socket treatment and recovery time after removal were recorded,then were compared and analyzed. Results： Compared with inflammation group,there were significant statistical difference on postoperative bleeding tooth extraction,facial swelling,pain,mouth opening and healing treatments in group without inflammation and relieve inflammation group（ P〈0. 05）,but the difference in the incidence of dry socket was not statistically significant. Conclusion： Selecting none inflammation and inflammation remission tooth to pull tooth extraction can improve postoperative bleeding,facial swelling,local pain and mouth opening,shorten recovery treatment,but the inflammation of the tooth extraction does not increase the incidence of dry socket.
