肱骨远端Y型钢板内固定治疗肱骨髁间骨折的临床研究

Clinical Research of Patients with Humeral Intercondylar Fracture Treated by Type Y Distal Humeral Plate Internal Fixation

目的:探讨肱骨远端Y型钢板内固定治疗肱骨髁间骨折的临床效果。方法:将我院2009年9月至2013年9月收治的肱骨髁间骨折患者100例随机分为研究组(n=50)和对照组(n=50),分别接受肱骨远端Y型钢板和双钢板内固定治疗,比较两组治疗效果。结果:研究组的固定成功率明显低于对照组(86%vs 94.0%,P<0.05),但两组患者的手术时间、术中出血量、及术后1年的Harris评分无明显差异(P>0.05),而研究组患者术后2年的Harris评分显著高于对照组(P<0.05)。结论 :肱骨远端Y型钢板内固定治疗与双钢板内固定治疗肱骨髁间骨折均可取得满意度内固定效果,虽然Y型钢板固定成功率较低,但其远期疗效好,临床可根据患者实际情况选择合适的内固定方式。

Objective：To explore the clinical effect of humeral intercondylar fracture treated by type Y distal humeral plate internal fixation. Methods：100 patients with humeral intercondylar fracture were randomly divided into research group（n=50） and control group（n=50）from Sep 2009 to Sep 2013, and were received type Y distal humerus plate internal fixation and double plate accept double plate internal fixation respectively. The clinical effects were compared between the two groups. Results：The success rate of fixation in research group was significantly lower than that in the control group （86% vs 94%,P〈0.05）. Operation time,amount of bleeding during and after the operation, and Harris score after 1 years were no significant difference between the two groups （all P〉0.05）. But Harris score after 2 years in research group was significantly higher than that in the control group （P〈0.05）. Conclusion：Type Y distal humerus plate internal fixation treatment with double plate internal fixation for the treatment of humeral intercondylar fractures can obtain satisfaction effect of internal fixation of Y shaped steel plate,although the success rate is low,but its long-term effect is good,proper selection of internal fixation methods according to the actual situation of patients with clinical need.
