彩色多普勒超声诊断乳腺肿瘤的临床诊断价值 被引量:4

摘要 目的探讨彩色多普勒超声在乳腺良恶性肿瘤鉴别诊断中的应用价值。方法 80例乳腺肿瘤患者作为研究对象,经病理穿刺实验证实乳腺癌患者42例,纤维腺瘤患者38例。对所有患者行乳腺彩色多普勒超声检查,观察病灶血流信号、波动指数(PI)、阻力指数(RI)、峰值流速等血流动力学指标,并进行良恶性质鉴别诊断,与病理证实结果比较。结果彩色多普勒超声对乳腺癌及纤维腺瘤的鉴别诊断结果与病理证实结果比较,差异无统计学意义(P>0.05);良性肿瘤与恶性肿瘤的血流信号丰富程度、血流峰值流速、RI、PI指标比较,差异有统计学意义(P<0.05)。结论采用彩色多普勒超声检查乳腺肿瘤具有较高的良恶性鉴别能力,可准确检出良恶性肿瘤病灶血流动力学指标差异,同时该检查为无创性检查,较穿刺活检具有方便、快捷、经济的优势,具有临床应用及推广价值。 ObjectiveTo investigate application value of color Doppler ultrasound in differential diagnosis of benign and malignant breast tumor.MethodsThere were 80 patients with breast tumor as study subjects. Among them, there were 42 cases with breast cancer under pathological biopsy, and 38 cases with fibroadenoma. All patients received color Doppler ultrasound for breast examination in observation of hemodynamic indexes, as blood flow signal, fluctuation index (PI), resistance index (RI), peak blood flow. Differential diagnosis was made for benign and malignant breast tumor, comparing with pathological confirming results.ResultsThere was no statistically significant difference of breast cancer and fibroadenoma diagnosis results between color Doppler ultrasound differential diagnosis and pathological confirming results (P〉0.05). The differences of rich blood flow signal, peak blood flow, RI and PI between benign and malignant tumors (P〈0.05). ConclusionImplement of color Doppler ultrasound provides high differential effect for benign tumor in breast tumor. This method is a non-invasive examination with accurate diagnosis of hemodynamic difference between benign and malignant tumor. It contains more advantages of convenience, fast speed and economy than puncture biopsy, and it provides value for clinical application and promotion.
