结直肠癌手术并发症的临床分析(附110例报告)

The clinical analysis of colorectal cancer surgery complications

目的分析结直肠癌术后常见并发症的发生情况。方法系统性回顾分析2007年1月至2014年1月在本院行结直肠癌根治术的110例患者的临床资料,黏液腺癌11例,中分化腺癌95例,低分化腺癌3例,绒毛状腺瘤癌变1例;按手术后的肿瘤病理(American Joint Committee on Cancer,AJCC)分期,I期16例,Ⅱ期47例、Ⅲ期43例,Ⅳ期4例。合并有Ⅱ型糖尿病9例,高血压病14例,术前不全性肠梗阻3例。结果 110例中发生吻合口漏4例(发生率为3.64%);出血4例(发生率为3.64%),其中术中出血1例,术后出血3例;感染7例,其中肺部2例(发生率为1.82%);以及术后肠梗阻5例(发生率为4.55%)。结论只要围手术期处置得当,可以减少并发症的发生率。

Objective To analyze the common complications of colorectal cancer surgery.Methods The clinic data of 110 patients underwent colorectal cancer surgery from January 2007 to January 2014 was analyzed.The cases were excluded under the conditions of acute intestinal obstruction,intra-abdominal spread,lungs/brain/bone metastases,ASA（American Society of Anesthesiologists）risk Ⅳ and Ⅴ.There were eleven mucinous adenocarcinoma cases,ninety-five differentiated adenocarcinoma cases,three poorly differentiated adenocarcinoma cases and one villous adenocarcinoma canceration.According to the postoperative pathologic（AJCC）,there were sixteen cases on phase I,forty-seven cases on phaseⅡ,forty-three cases on phaseⅢ and four cases on phaseⅣ.All the datas were analyzed by SPSS 13.0using X2 test for association.Results Four patients occurred anastomotic leakage（3.64%）;Five patients suffered from bleeding（3%）;Seven patients were infected（2with lung infection,3.64%）;and 5patients ileus in 4to 13 days after operation（4.55%）.Conclusion As long as disposing properly,can reduce the incidence of complication.
