摘要
目的探讨腹腔镜和开腹结直肠癌根治术患者外周血中鸟苷酸环化酶C(GCC)mRNA和人端粒酶逆转录酶(hTERT)mRNA的表达及意义。方法按照随机数字表法将2011年7月至2013年7月我院收治的结直肠癌患者分为腹腔镜组和开腹手术组,通过逆转录-聚合酶链反应技术(RT-PCR)检测并比较两组患者外周血GCC mRNA和hTERT mRNA的表达情况。结果腹腔镜组手术时间长于开腹手术组,差异有统计学意义(P<0.05),术中出血量、术后排气时间、住院时间均少于开腹组,差异有统计学意义(P<0.05);腹腔镜组和开腹组患者手术前、手术后的GCC mRNA、hTERT mRNA阳性率表达分别比较差异无统计学意义(P>0.05),两组患者手术后的GCC mRNA、hTERT mRNA阳性率均高于手术前,差异有统计学意义(P<0.05)。结论结直肠癌采用腹腔镜和开腹手术根治治疗均会增加血行微转移可能性,但是腹腔镜结直肠根治术不会增加术后肿瘤血循环微转移的危险性。
Objective To investigate the expression and significance of Guanylyl cyclase C(GCC)mRNA and human telomerase reverse transcriptase(hTERT)in the peripheral blood in colorectal cancer patients with laparoscopic and open radical resection.Methods Patients with colorectal cancer in our hospital from July 2011 to July 2013 were divided into laparoscopic group and laparotomy operation group according to the random number table method.Reverse transcriptase polymerase chain reaction(RT-PCR)was used to detect the expression of GCC mRNA and hTERT mRNA in peripheral blood,and the expression of GCC mRNA and hTERT mRNA between the two groups were compared.Results The operation time of laparoscopic group was more than that of open operation group,and the amount of intraoperative bleeding,postoperative exhaust time,hospitalization time were less than those of open surgery group,with the difference was statistically significant(P〈0.05).The positive expression rate of GCC mRNA and hTERT mRNA before and after operation in two groups were no statistically significant differences(P〉0.05).The positive rate of GCC mRNA,hTERT mRNA after operation in two groups were higher than those before the operation,and the difference was statistically significant(P〈0.05).Conclusion Using laparoscopic and open radical operation treatment for colorectal cancer will increase the likelihood of blood micrometastasis,but laparoscopic colorectal resection dose not increase the risk of postoperative micrometastasis of tumor blood circulation.
