摘要
目的探讨患者肾功能状态对血清人附睾蛋白4(HE4)水平及ROMA指数的影响。方法通过检测160例非肿瘤但肾功能受损的患者血清HE4、糖类抗原125(CA125)、尿素(Urea)、肌酐(CRE)、胱抑素C(Cys C)水平,计算患者ROMA指数和内生肌酐清除率(Ccr),比较不同肾损害分组间的血清HE4水平、CA125水平、ROMA指数的差异。结果血清HE4水平、ROMA指数与患者血清Urea、CRE、Cys C水平呈正相关(P=0.000),与Ccr呈负相关(P=0.000),而血清CA125水平与患者血清Urea、CRE、Cys C、Ccr相关性不明显(P>0.05)。随着肾损害的分期增加,患者血清HE4水平、ROMA指数随之升高,各组间差异有统计学意义(P<0.05),而CA125水平无明显变化(P>0.05)。当男性与女性组之间年龄、Ccr差异无统计学意义时,两组间血清HE4水平、CA125水平、ROMA指数比较,差异无统计学意义(P>0.05)。结论患者肾功能状态对血清HE4水平和ROMA指数有明显的影响,在利用血清HE4和ROMA指数进行卵巢癌的辅助诊断和疗效评估时,要充分考虑到肾脏因素对血清HE4水平和ROMA指数的影响。
Objective To explore the effect of the renal function on serum HE4 level and ROMA index. Methods By detecting the serum levels of human epididymal protein 4( HE4),carbohydrate antigen 125( CA125),urea( Urea),creatinine( CRE) and cystatin C( Cys C) in 160 non- tumor patients with a renal damage,to calculate the ROMA index and endogenous creatinine clearance rate( Ccr),and compare the distinctions of serum HE4 level,CA125 level and ROMA index among the groups of different renal damage. Results The serum HE4 level and ROMA index were positively correlated to the serum level of Urea,CRE and Cys C( P = 0. 000),but negatively correlated to the Ccr( P = 0. 000); and the serum CA125 level was not obviously correlated to the serum level of Urea,CRE,Cys C and Ccr( P〉0. 05). The serum HE4 level and ROMA index increased with the stage of renal damage,and the differences among different groups were statistically significant different( P〈0. 05),but the CA125 level did not obviously change( P〉0. 05). When the differences of age and Ccr in the male group and the female group were not statistically significant,the differences of serum HE4 level,CA125 level and ROMA index were not statistically significant( P〉0. 05). Conclusion The serum HE4 level and ROMA index could be obviously influenced by the state of renal function,and thus their effect should be fully considered while applying for auxiliary diagnosis and efficacy assessment of ovarian cancer.
