菌血症患者中性粒细胞VCS参数的变化及意义

Changes and significance of VCS parameters of neutrophil in patients with bacteremia

目的探讨菌血症患者中性粒细胞VCS参数的变化及意义。方法采用BECKMAN COULTERLH750血细胞分析仪检测51例菌血症患者及64例健康体检者的白细胞数及中性粒细胞VCS参数,分析菌血症患者治疗前、后白细胞数、中性粒细胞平均体积(MNV)、平均电导率(MNC)、平均激光散射值(MNS)和体积分布宽度(NDW)参数的变化。结果菌血症组(治疗前)与正常对照组比较WBC、MNV、MNC、NDW增高,MNS降低,差异有统计学意义(P<0.05);菌血症组(治疗后)与正常对照组比较各参数差异无统计学意义(P>0.05)。WBC≤10×109/L并且NE≤70%组和WBC>10×109/L并且NE>70%组治疗前VCS参数与正常对照组比较均有差异,差异有统计学意义(P<0.05);治疗后与正常对照组比较差异无统计学意义(P>0.05)。结论中性粒细胞的VCS参数能反映菌血症患者血液中性粒细胞的形态学变化,对菌血症患者的诊疗具有一定的临床指导意义。

Objective To explore the changes and the clinical significance of VCS parameters of neutrophil in patients with bacteremia. Methods The WBC and VCS parameters of neutrophil（ NE） of 51 cases bacteremia patients and 64 cases health controls were examined with Beckman Coulter LH750 hematology analyzer,and the changes of neutrophil volume（ MNV）,mean channel of neutrophil conductivity（ MNC）,mean channel of neutrophil scatter（ MNS）,neutrophil volume distribution width（ NDW） were analyzed. Results Compared with control group,the WBC,MNV,MNC and NDW of bacteremia group（ before treatment） increased and MNS decreased,and the difference was statistically significant（ P〈0. 05）. The differences on the results of WBC,MNV,MNC MNS and NDW of bacteremia group（ after treatment） were not statistically significant,compared with the control group（ P〈0. 05）. The VCS parameters of neutrophil（ NE） of the WBC≤10 × 10^9/ L with NE≤70% group and the WBC〉 10 × 10^9/ L with NE 70% group were significantly different from that of the control group before treatment,and the difference had statistical significance（ P〈0. 05）,and compared with control group（ after treatment） after treatment,the difference had no statistical significance（ P〉0. 05）. Conclusion The neutrophil VCS parameters can reflect the morphologic changes of neutrophil in patients with bacteremia,and has certain clinical guiding significance for the treatment of bacteremia patients.
