摘要
目的了解太原地区乙肝病毒感染者HBV基因型分布情况,分析HBV基因型与HBe Ag、HBV DNA载量等临床指标之间的关系。方法采用TRFIA技术检测103例乙肝病毒感染者的血清标志物;实时荧光法检测HBV基因型;荧光定量PCR法测HBV DNA载量;统计学分析HBV基因型与其临床指标之间的相关性。结果所有感染者中,94例被分型,C型占70.2%,B型占24.5%。HBe Ag阳性56例,HBe Ag阴性38例,其中B型感染者HBe Ag阳性有7例(30.4%),C型48例(72.7%);HBV DNA高载量感染者共检出62例,B型检出率为65.2%,C型检出率为66.7%;统计学结果表明HBV基因分型与感染者的性别、年龄差异无统计学意义(P>0.05),但与HBe Ag及HBV DNA载量差异有统计学意义(P<0.05)。另大三阳感染者中47例为HBV DNA高载量,检出率显著高于小三阳感染者。结论太原地区乙肝病毒感染者HBV基因型以C型为主,且基因分型与HBe Ag和HBV DNA载量有一定相关性。
Objective To understand the distribution of HBV genotype in hepatitis B patients in Taiyuan area,and analyze the relationship of clinical indexes among the HBV genotype,HBe Ag and HBV DNA contents. Methods The HBV serum markers of 103 cases of hepatitis B patients were detected by TRFIA; The detection of HBV genotyping were conducted by real- time fluorescence; and fluorescent quantitative PCR was used to detect HBV DNA contents. The correlations between HBV genotypes and clinical indexes were statistically analyzed. Results 94 cases of hepatitis B patients were genotyped,of which genotype C was 70. 2%,genotype B was 24. 5%. 56 cases with HBe Ag positive specimens and 38 cases with HBe Ag negative were detected,of which genotype B in HBe Ag positive were 7 cases( 30. 4%),genotype C 48 cases( 72. 7%); 62 cases of high HBV DNA contents were detected,in which the rate of genotype B was 65. 2%,and the detection rate of genotype C was 66. 7%.And statistical results showed that the differences among the HBV genotype,gender and age of hepatitis B patients had no statistical significance( P〉 0. 05),but the differences between the HBV genotype and HBe Ag and HBV DNA contents were statistically significant( P 〈0. 05). While,47 cases with higher HBV DNA contents were found in the big 3 positive,the detection rate was significantly higher than that of the small 3 positive. Conclusion Genotype C is the dominant for HBV genotype in Taiyuan,and there are some correlations with HBe Ag and HBV DNA contents.
