摘要 目的:观察CPM早期功能锻炼对胫骨平台Schatzker II型骨折术后膝关节功能恢复的影响。方法:回顾分析本科收治并手术治疗的胫骨平台Schatzker II型骨折患者62例,其中术后2周内采用CPM被动功能锻炼患者32例(观察组),术后采用主动功能锻炼进行康复训练的患者32例(观察组),观察两组术后3、5、7、14d及1个月时膝关节功能恢复情况。结果:观察组术后第3天疼痛缓解明显优于对照组,但术后第5天疼痛缓解明显差于对照组,差异具有统计学意义(P<0.05);观察组术后各时间点膝关节活动度均明显高于对照组,差异具有统计学意义(P<0.05);两组术后第3天、1个月时股四头肌肌力比较,差异无统计学意义(P>0.05);观察组术后第5、7、14天股四头肌肌力均明显高于对照组,差异具有统计学意义(P<0.05);两组术第3天膝关节功能恢复比较,差异具有统计学意义(P>0.05);观察组术后第5、7、14天及1个月时膝关节功能恢复程度均明显优于对照组,差异具有统计学意义(P<0.05)。结论:CPM早期功能锻炼可有效增强胫骨平台Schatzker II型骨折术患者股四头肌肌力,改善膝关节活动度,对术后膝关节功能恢复促进作用显著。 Objective To explore effect of early passive exercise by CPM on patients with tibial plateau Schatzker II fracture after surgery. Methods: Clinical data of 60 cases with tibial plateau Schatzker II fracture receiving surgery were ana lyzed retrospectively, including 32 cases with early passive exercise by CPM (observation group) and 32 cases with active exercise (control group). The rehabilitation status of knee joint 3 d, 5 d, 7 d, 14 d and 1 month after surgery was observed. Resuits Three days after surgery, the relieving effect of observation group was more significant than that of control group, but the effect was weaker on the 5th day (P(0.05). Mobility of knee joint of observation group was higher than that of control group at all time points (P〈0.05). Myodynamia of quadriceps femoris was stronger in observation group than that of control group 5 d, 7 d, 14 d after surgery (P〈0.05). Recovery degree of knee joint was more significant in observation group 5 d, 7 d, 14 d and 1 month (P〈0.05). Conclusions.. Early passive exercise by CPM can effectively strengthen myodynamia of quadri- ceps femoris, improve mobility of knee joint, and can effectively promote function recovery.
