空腹血糖受损人群血清微量元素与胰岛β细胞功能及胰岛素敏感性的相关性研究 被引量:2

STUDY ON THE CORRELATION OF SERUM TRACE ELEMENTS AND PANCREATIC BETA CELL FUNCTION AND INSULIN SENSITIVITY IN INDIVIDUALS WITH IMPAIRED FASTING GLUCOSE

摘要 目的 探讨空腹血清受损(ISF)人群血清微量元素Cu、Zn、Se含量和Cu/Zn比与胰岛β细胞功能及胰岛素敏感性之间的相关性。方法 采用整群抽样结合系统抽样方法对在宁夏回族自治区吴忠市和固原市抽取的1267名25-74岁农村居民进行问卷调查和体格检查,并测定有关生化指标及血清中微量元素含量;采用HOMA-2模型计算胰岛β细胞功能参数(%β)、胰岛素敏感性参数(%S)和胰岛素抵抗参数(IR)。结果 共1267例研究对象,其中约55%(n=698)为NGT人群,约45%(n=569)为IFG人群。NGT与IFG组间微量元素的差异无统计学意义;NGT与IFG组比较,IFG组的%β、%S明显降低(P〈0.001),IR增加(P〈0.05);多元线性结果表明,Cu、Zn、Cu/Zn、腰臀比(WHR)与%β存在相关性。结论 与NGT组比较,IFG组出现明显的%β和%S降低,IR增加;Cu、Zn、Cu/Zn、WHR与%β存在相关性。 Objective To test if there are differences in serum zinc, copper, selenium concentrations and Cu/Zn ratio between normal glucose tolerance (NGT) and impaired fasting glucose (IFG) groups; and if these parameters are associated with pancreatic beta cell function and insulin sensitivity. Methods One thousand two hundred and sixty seven rural residents aged 25 to 74 were selected from four villages in Guyuan and Wuzhong cities in Ningxia Province by stratified cluster sampling combined with systematic sampling. Questionnaire survey, physical examination and the biochemical tests were carried out and the levels of serum trace elements were measured. Beta cell function (%β), insulin sensitivity (%S) and insulin resistance (IR) were calculated for NGT and IFG participants using the Homeostasis Model Assessment (HOMA-2) calculator. Results Approximately 55% (n=698) were NGT; 45% (n=569) were IFG. Serum trace elements were not significantly different between NGT and IFG groups. However, HOMA-2 parameters were significantly different between NGT and IFG groups, in which %β and %S in IFG group were lower than NGT group (P〈0.001= and IR was higher than NGT group (P〈0.05=. The multiple linear regression for %β showed a statistically significant association between serum zinc, copper, Cu/Zn ratio and waist-hip ratio(WHR). ConclusionCompared with the NGT group, the IFG group has significantly decreased %β and %S, and significantly increased IR ; serum zinc, copper, Cu/Zn ratio and WHR show a statistically significant association with %β.
