围手术期输血对肝恶性肿瘤术后复发率的影响

Effect on liver cancer recurrence rate of perioperative blood transfusion

目的:探讨围手术期输血对肝恶性肿瘤术后远期复发率及近期并发症的影响。方法:回顾性分析90例肝恶性肿瘤手术患者,按照有无输血分为2组,每组45例,对其临床资料进行分析。结果:1随访90例肝恶性肿瘤手术患者显示35例患者复发,其中输血组有25例(71.4%)患者复发,无输血组有10例(28.6%)复发,输血组患者术后复发显著高于无输血组(P<0.05),多元回归分析显示,TNM分期、淋巴结转移、围手术期输血均为影响患者预后的独立危险因素。2输血组患者肿瘤大小、侵袭度、淋巴结转移、TNM分期与无输血组比较,差异有统计学意义(P<0.05)。结论:肝恶性肿瘤患者术后复发率及相关并发症的发生与围术期输血存在着密切的关系,围手术期输血可作为判断肝恶性肿瘤远期复发的危险因素。

Objective：To investigate the impact of perioperative blood transfusion on short-term complications and long-term recurrence rate of liver cancer surgery.Method：A retrospective analysis of 90 cases of liver cancer surgery patients,divided into two groups according to the presence or absence of blood transfusion clinical data,were analyzed.Result：1Follow-up showed 35 cases of relapse,including 25 cases of transfusion group（71.4%）,10cases（28.6%）of no transfusion group,and recurrence transfusion patients was significantly more than nontransfusion group（P〈0.05）.Multiple regression analysis showed that lymph node metastasis,TNM stage,perioperative transfusion were independent risk factors for the prognosis of patients.2The tumor size,degree of invasion,lymph node metastasis and TNM stage in transfusion group were significantly higher than those in nontransfusion group（P〈0.05）;Conclusion：Postoperative recurrence rate and incidence of related complications of patients with hepatocellular carcinoma may be closely related to the perioperative blood transfusion,and perioperative blood transfusion might be as an independent risk factor for the judgment of liver cancer recurrence.
