锁定钢板固定配合中药熏洗治疗股骨髁间髁上骨折的疗效分析

Effects of locking plate fixation plus TCM washing on femoral condyle fracture

目的：观察锁定钢板内固定配合中药熏洗治疗股骨髁间、髁上粉碎骨折的疗效。方法：2008年6月-2014年5月,采用锁定钢板配合中药熏洗治疗股骨髁间、髁上粉碎骨折患者52例,男38例,女14例;年龄19~67岁,中位数35岁;左侧31例,右侧21例。根据AO股骨远端骨折分类C2型35例,C3型17例。术后随访观察刀口愈合、骨折愈合、并发症发生及患肢功能恢复情况。结果：本组52例,术后X线片示骨折解剖复位33例,功能复位19例,固定稳定无松动,切口均一期愈合。52例均获随访,随诊时间5~18个月,平均11个月,未发生畸形愈合、骨不连及断钉等并发症。按Shelboume和Brueckman评分标准评定,优38例,良11例,可3例,优良率为94%。结论：采用锁定钢板固定配合中药熏洗治疗股骨髁间、髁上粉碎骨折,固定牢靠,并发症少,能进行早期功能锻炼。配合中药活血化瘀,松解粘连,滑利关节,使骨折愈合率高,关节功能恢复满意,值得临床推广应用。

Objective： To observe therapeutic effects of locking plate internal fixation plus TCM medicine washing on femoral condyle and condylar comminuted fracture. Methods： Condylar fracture in 52 cases, male 38 cases, female 14 cases. Age 19 ~ 67 years old, a median of 35 years were included. Left side in 31 cases, right side in 21 cases. According to the classification of AO type C2 in 35 cases, 17 cases of type C3. Wound healing, fracture healing, complications and functional recovery of limb fracture were observed. Results： In 52 cases, postoperative X-ray film showed fracture anatomical reduction in 33 cases, functional reduction in 19 cases, stable and stable without loosening, incision uniform healing. 52 cases were followed up for 5 to 18 months, for an average of 11 months. According to Shelboume and Brueckman score, 38 cases were excellent; 11 cases were good; 3 cases were better; the effciency was 94%. Conclusion： Treating femoral condyle and condylar fracture by the locking plate fixation plus TCM washing, with less complications, is effective.
