G6PI与RF联合检测在类风湿性关节炎诊断中的应用

The application of G6PI and RF combined detection in the diagnosis of rheumatoid arthritis

目的探讨血清中葡萄糖-6-磷酸异构酶(G6PI)和类风湿因子(RF)对类风湿性关节炎的诊断价值。方法采用酶联免疫吸附试验(ELISA)及免疫比浊法分别测定105例RA患者、25例创伤性关节炎、70例健康体检者的血清标本中G6PI、RF的表达水平。结果 G6PI和RF在诊断RA中的敏感度分别为76.2%和83.8%,特异度分别为92.6%和80.0%,两者联合检测敏感度和特异度分别为90.2%和87.5%;105例RA患者血清G6PI、RF含量分别为(5.12±7.93)μg/ml、(25.3±41.2)IU/ml,显著高于创伤性关节炎组(0.12±0.10)μg/ml、(7.5±10.5)IU/ml和健康对照组(0.25±1.12)μg/ml、(2.5±14.1)IU/ml,差异有统计学意义(P<0.01)。结论 G6PI在RA患者血清中高度表达,并且与疾病活动性有密切关系,有望成为RA的新型标志物,其与传统诊断项目RF联合检测可以很大程度地提高RA诊断的敏感度与特异度。

Objective To investigate diagnostic significance of G6 PI and RF in serum with rheumatoid arthritis. Methods The serum expression level of G6 PI and RF in serum samples from 105 RA patients,25 traumatic arthritis patients and 70 healthy donors were tested by ELISA and immunonephelometry. Results In the RA patients,the sensitivity of G6 PI and RF were76. 2% and 83. 8%,respectively,the specificity were 92. 6% and 80. 0%,respectively,and the sensitivity and specificity of combined detecting G6 PI and RF were 90. 2% and 87. 5%,respectively; the level of G6 PI and RF in 105 RA patients serum were（ 5. 12 ± 7. 93） μg / ml,（ 25. 3 ± 41. 2） IU / ml,respectively,which were significantly higher than that of the traumatic arthritis patients（ 0. 12 ± 0. 10） μg / ml,（ 7. 5 ± 10. 5） IU / ml,and that of the healthy donors（ 0. 25 ± 1. 12） μg / ml,（ 2. 5 ±14. 1） IU / ml,and the difference had statistical significace（P〈0. 01）. Conclusion The high expression of G6 PI in RA patients was correlated with the activity of RA,and G6 PI may be a new mark of RA. And the sensitity and specitivity will be improved largely if combined detection of RF with traditional diagnostic projects.
