摘要
目的评估分析TEG5000血栓弹力图仪(TEG仪)主要参数性能,以验证其是否符合临床实验室的检测要求。方法按标准操作程序(SOP)对TEG仪的主要参数,如从血样开始检测至描记图幅度达2 mm所需的时间(R时间,R)、从R时间终点至描记幅度达20 mm所需时间(K时间,K)、从血凝块形成点至描记图最大曲线弧度作切线与水平线的夹角(α角度,α)、描记图上的最大振幅即最大切应力系数(MA)、凝血综合指数(CI)、任一时刻描记图曲线两点间的距离(A)和血凝块强度(G)等进行重复性、线性、可比性试验,同时观测其对血小板(PLT)和纤维蛋白原(Fbg)的影响。结果重复测定全血标本R、K、MA、CI等主要参数的CV范围为1.8%~11.1%;线性试验R等的相关系数(r)绝对值均〉0.95,表明仪器的实测值和理论值具有良好的相关性;R和MA与PT、APTT、TT和Fbg呈正相关(P〈0.01)。PLT和Fbg的量对血栓弹力图各参数的影响不同,根据参数的变化可以看出标本中PLT和Fbg的变化。结论 TEG仪检测主要参数性能符合要求,可推广应用于临床。但应注意PLT和Fbg对其测定结果的影响。
Objective To evaluate the main performance parameters of thromboelastography TEG5000,and to test if it meets the examination requirements of clinical laboratory test. Methods According to standard operational procedure( SOP),the main parameters of the TEG were detected from the blood samples to the time reaching to the 2 mm( R) required time( R time,R),the time from the end of R to the graph amplitude reaching 20 mm( K time,K),from the blood clot formation point to the maximal curve radian as the angle of tangent and horizontal line( α angle,α),the maximal amplitude,namely the maximal coefficient of the shear stress( MA),coagulation index( CI),the distance between two point( A) at any time in the graph,and blood clot strength( G) were tested to show the repeatability,liner and comparability. The influence of platelet( PLT) and fibrinogen( Fbg) were also observed at the same time. Results The main parameters of K,MA,CI and R of CV were in the range of1. 8%- 11. 1%. The absolute value of the correlation coefficient( r) of linear test R was more than 0. 95,showing that the instrument has a good correlation with the measured value and theoretical value. The correlation between R,MA and PT,APTT,TT and Fbg was positive(P〈0. 01). The influences of platelets and fibrinogen on thromboelastography parameters were different,and the changes of PLT and Fbg in samples could be identified according to the parameters changing. Conclusion The main performance parameters of TEG meets the requirement of clinical work,which can be promoted. But the influence effect by PLT and Fbg should also be concerned.
