复方阿胶浆提高排卵障碍性不孕患者妊娠率的疗效和机理研究

Ejiao Composite Syrup increases pregnancy rate in women with ovulation obstructive infertility: efficacy and mechanistic study

目的:研究复方阿胶浆联合克罗米芬提高排卵障碍性不孕患者妊娠成功率的作用及机理。方法:将150例患者随机的分为中西医结合治疗组即复方阿胶浆联用克罗米芬组(Ⅰ组)、西药对照组即克罗米芬组(Ⅱ组)及中药对照组即复方阿胶浆组(Ⅲ组)3组,监测服药后性激素水平和宫颈粘液变化、子宫内膜厚度及类型、子宫及卵巢动脉血流、卵泡成熟及排卵妊娠等指标。结果:Ⅰ组各项监测指标均明显优于Ⅱ组、Ⅲ组,差异有统计学意义(P<0.05)。Ⅱ组卵泡成熟及排卵妊娠率优于Ⅲ组,差异有统计学意义(P<0.05)。Ⅲ组子宫及卵巢动脉血流优于Ⅱ组,差异有统计学意义(P<0.05)。而Ⅱ组、Ⅲ组FSH、LH、E2、P水平;宫颈Insler评分;子宫内膜厚度及A型内膜比较差异无统计学意义(P>0.05)。结论:复方阿胶浆联合克罗米芬可更有效地调节体内性激素水平,促进卵泡发育,健全黄体,促进排卵,改善子宫卵巢血供,增强子宫内膜容受性、显著提高妊娠率。

Objective： To investigate the efficacy and mechanism of Ejiao Composite Syrup in combination with clomiphene（ CC） in increasing the pregnancy rate among women with ovulation obstructive infertility.Methods： A total of 150 patients were randomly divided into the Integrated Chinese and Western Medicine group to receive Ejiao Composite Syrup and clomiphene（ Group Ⅰ）,or the western medicine control group to receive clomiphene alone（ group Ⅱ）,or Chinese medicine（ TCM） control group to receive Ejiao Composite Syrup alone（ group Ⅲ）. After medication,the patients were monitored for changes in the sex hormone levels,cervical mucus,endometrial thickness and types,uterine and ovarian blood flow,maturity of follicles and ovulation pregnancy.Results： Patients in group I showed favorable indicators compared with those in groups II and III（ all P 0.05）.Patients in group II showed better maturity of follicles and rate of ovulated pregnancy than those in group III（ P〈0.05）. Patients in group III showed better blood flow of the uterine and ovaries than those in group II（ P〈0.05）.Between groups II and III,there were no significant differences in FSH,LH,E2,and P levels,cervical Insler score,endometrial thickness and type A lining（ P 〉0. 05）. Conclusion： Ejiao Composite Syrup in combination with clomiphene can effectively regulate the sex hormone levels,promote follicular development,improve the corpus luteum,stimulate ovulation,improve uterine and ovarian blood supply,enhance the receptivity of endometrium,and significantly increase the rate of pregnancy.
