摘要
目的研究CTL、NK及TIDC在胃腺癌组织中的分布情况及对病人生存与预后的影响。方法将CD8作为细胞毒T淋巴细胞(CTL)的标记物,CD56作为自然杀伤细胞(NK)的标记物,S-100作为肿瘤浸润树突状细胞(TIDC)的标记物,应用免疫组织化学方法检测肿瘤组织中CTL、NK及TIDC的分布和浸润情况。结果 80例胃腺癌中,CTL无-轻度浸润的48例,5年生存率为38%;显著浸润的32例,5年生存率为63%;NK细胞无-轻度浸润的50例,5年生存率42%,显著浸润的30例,5年生存率为63%;TIDC无-轻度浸润的52例,5年生存率为37%;TIDC显著浸润的28例,5年生存率为57%;NK/T及TIDC同时显著浸润的24例,5年生存率为67%;无-轻度浸润的32例,5年生存率为47%,经Log-rank检验,NK/T及TIDC显著浸润组和无-轻度浸润组的5年生存率差异有统计学意义(P=0.00)。结论胃腺癌组织中CTL、NK及TIDC显著浸润组的预后和生存时间明显好于轻度浸润组。
Objective To study the distribution and effect of CTL, NK and TIDC in gastric adenocareinoma patients on the survival and prognosis of the patient. Methods The distribution of the markers CD8, CD56 and S- 100 of cytoxic lymphocyte (CTL), natural killer (NK) and tumor Infiltrating dendritic cell (TIDC) in 80 patients with gastric adenocarcinoma were detected immunohistoehemieally. Results CTL infiltration in 48 gastric adenocareinoma patients was zero or mild with the five-year survival rate of 38%, while CTL infiltration was marked in 32 eases with the five-year survival rate of 63%. The NK infiltration in 50 gastric adenoeareinoma eases was zero or mild with the five-year survival rate of 42%, while the NK infiltration in 30 gastric adenocareinoma eases was marked with the five-year survival rate of 63%. TIDC infiltration in 50 gastric adenoeareinoma eases was zero or mild with the five-year survival rate of 37% , while it was marked in 28 gastric adenoeareinoma eases with the five-year survival rate of 57% .The CTL and the NK and the TIDC cells in 32 gastric adenoeareinoma cases were zero or mild with the five-year survival rate of 47%, while it was marked in 24 eases with the five- year survival rate of 67%.The five-year survival rates between marked and zero or mild infiltration groups were significantly different (P=0.0) by Log-rank test. Conclusion The prognosis and survival time of gastric adenoearcinoma eases with marked CTL, NK and TIDC infiltration were significantly better than that in mild infiltration group.
