葶苈大枣泻肺汤加减治疗恶性胸腔积液的临床观察与分析

Clinical observation on treating malignant pleural effusion with the Tingli Dazao Xiefei decoction

目的:观察分析葶苈大枣泻肺汤治疗恶性胸腔积液的临床效果。方法:108例随机分为观察组和对照组,两组均采用细管胸腔闭式引流。观察组采用中药针剂香菇多糖胸腔内灌注、煎服葶苈大枣泻肺汤加减;对照组采用顺铂胸腔内灌注。结果:观察组CR 23例(42.59%),PR 21例(38.89%),SD 3例(5.56%),PD 7例(12.96%),RR 44例(81.48%)。对照组CR 14例(25.93%),PR 20例(37.04%),SD 5例(9.26%);NC 15例(27.77%),RR 34例(62.97%)。经统计学处理,两组在CR、RR有显著性差异(P<0.05)。结论:葶苈大枣泻肺汤加减治疗恶性胸腔积液可提高疗效,减低毒性反应。

Objective： To observe clinical effects of the Tingli Dazao Xiefei decoction on malignant pleural effusion. Methods： 108 cases were randomly divided into the observation group and the control group; two groups were treated by closed thoracic drainage. The observation group was treated with TCM medicine injection of lentinan intrathoracic instillation, with the Tingli Dazao Xiefei decoction; the control group was treated by cisplatin intrapleural instillation. Results： In the observation group, 23 cases were CR （42.59%）; 21 cases were PR （38.89%）; 3 cases were SD （5.56%）, and 7 cases were PD （12.96%）. 44 cases were RR （81.48%）; in the control group, 14 cases were CR （25.93%）; 20 cases were PR （37.04%）; 5 cases were SD （9.26%）;15 cases were NC （27.77%）; 34 cases were RR （62.97%）. The difference of CR and RR in two groups were significant （P〈0.05）. Conclusion： Treating malignant pleural effusion with the Tingli Dazao Xiefei decoction can improve efficacy, reduce toxicity.
