摘要
目的探讨瞬时受体电位离子通道(TRPM7)在胆管癌组织中的表达及其与预后的相关性。方法应用免疫组织化学SP法检测TRPM7在49例胆管癌、7例胆管良性病变组织和36例癌旁正常胆管组织的表达情况,分析其与胆管癌临床病理的关系。结果胆管癌组织中TRPM7表达阳性率为77.6%(38/49),高于其在胆管良性病变组织的表达阳性率(0,0/7)及癌旁正常胆管组织中的表达阳性率(2.8%,1/36),差异均有统计学意义(P<0.05)。TRPM7蛋白的表达与胆管癌的TNM分期、淋巴结转移和脏器转移相关(P<0.05),但与患者的年龄、性别、部位、肿瘤分化程度及肝炎无相关性(P>0.05)。Kaplan-Meier生存分析发现TRPM7阳性表达患者的生存期低于TRPM7阴性表达患者(P<0.05)。Cox多因素分析显示,TRPM7的表达水平与生存预后显著相关(P<0.05),是影响预后的独立危险因素。结论 TRPM7在胆管癌的发生、发展和浸润转移中发挥着重要作用,是影响胆管癌患者预后的重要因素。
Objective To evaluate the expression of transient receptor potential melastatin 7(TRPM7) in cholangiocarcinoma and its correlation with prognosis. Methods The expressions of TRPM7 were detected by SP immunohistochemical in 49 cases of cholangiocarcinoma,7 cases of benign bile duct lesions and 36 cases of adjacent histologically noncancerous bile duct tissues,and to analysis its relationship with the clinical pathological characteristics of cholangiocarcinoma. Results The positive expression rate of TRPM7 in choIangiocarcinoma was 77.6 % (38/49), which was higher than that in benign bile duct lesions(0,0/7) and adjacent histologically noncancerous bile duct tissues(2.8 %, 1/36), the difference was statistically significant(P〈0.05). The positive expression of TRPM7 in cholangiocarcinoma was correlated with the TNM stage of tumor,lymph node metastasis and organ metastasis (P〈0. 05), but not related to patientsr age, gender, site, differentiation and hepatitis(P〉0.05). Kaplan-Meier analysis showed that increased expression of TRPM7 was associated with shorted overall survival (P〈0.05). Cox regression analysis showed that the expression level of TRPM7 was significantly associated with prognosis and an independent risk factor for prognosis(P〈0.05). Conclusion TRPM7 plays an important role in the tumorigenesis progression,invasion,and metastasis of eholangiocarcinoma, and is an important factor for prognosis in patients with cholangiocarcinoma.
