Expression of IL-23 in Patients with Bacterial Pneumonia and Its Correlation with Infection Severity

摘要 目的探讨IL-23在细菌性肺炎患者的表达及与感染程度的相关性。方法收集细菌性肺炎病例60例,根据入院时的痰培养及(或)痰涂片结果将其分为革兰阴性杆菌组(G-组,n=34)及革兰阳性球菌组(G+组,n=26)。另选择20名健康人作为对照组。测定3组患者外周血IL-23及降钙素原(PCT)的表达水平,并分析IL-23表达的差异及与PCT的相关性。结果与对照组比较,细菌性肺炎患者IL-23的表达显著升高(P<0.01),而G-组与G+组比较差异无统计学意义(P>0.05);G-组及G+组IL-23的表达与其PCT的表达水平呈正相关(r=0.682,r=0.656,均P<0.05)。结论细菌性肺炎患者的IL-23表达明显升高,其升高与病原菌种类无关,但能反映感染的严重程度。提示IL-23在细菌性肺炎的发病机制、疾病诊断中有一定的意义。 Objective To investigate the expression of interleukin-23(IL-23)in patients with bacterial pneumonia and its correlation with the severity of infection.Methods According to sputum culture and/or smear results,60 patients with bacterial pneumonia were divided into gram negative bacterium group(G- group,n=34)and gram positive coccus group(G+ group,n=26).In addition,20 healthy subjects were selected as the control group.The levels of IL-23 and procalcitonin(PCT)in peripheral blood were detected,and the correlation between IL-23 and PCT was analyzed.Results Compared with healthy controls,IL-23 expression significantly increased in patients with bacterial pneumonia(P〈0.01).No significant difference in IL-23 expression was found between G- group and G+ group(P〉0.05).There was a significant correlation between IL-23 expression and PCT expression in both G- group and G+ group(r=0.682 and r=0.656,respectively;P〈0.05).Conclusion The expression of IL-23 significantly increases in patients with bacterial pneumonia.The increase in IL-23 expression is not correlated with the pathogenic species but reflects the severity of infection,suggesting that IL-23 plays a role in pathogenesis and diagnosis of bacterial pneumonia.
