Constructing the system of hospital rational drug use according to evidence-based pharmacy

目的:以循证药学为基础构建符合医院实际的合理用药综合管理体系。方法:以临床药师为主导,以循证药学为基础建立新型基本药物遴选、临床路径、超说明书用药、处方点评、不良反应监测、抗菌药物管理、药学查房与会诊等多方面的运行制度,并以此构成医院合理用药综合管理体系。结果:武警江西总队医院通过3年的工作实践,医院合理用药各项指标呈明显改良趋势。结论:以循证药学为基础的医院合理用药体系能够有效发挥对临床用药的良好规范作用。

Objectives： To establish a comprehensive management system of hospital rational drug use according to evidence-based pharmacy. Methods： With the leading of clinical pharmacists＇ work, and according to evidence-based pharmacy, a new integrated management system of hospital rational drug use were established, which including selection of essential drugs, clinical pathway, over-label use, prescription review, monitoring of adverse reactions, antimicrobial drug management, pharmacy rounds and consultation. Results： Through three years of practices, the indicators of hospital rational drug use showed an upward trend. Conclusion： The system of hospital rational drug use which base in evidence-based pharmacy can play an effectively and normative role for the use of drug in clinical.
