摘要
目的对比SanderⅡ矫治器与肌激动器的制作、结构原理和临床效果,说明二者的异同点,进一步说明SanderⅡ矫治器治疗Ⅱ类Ⅰ分类错牙合的优点。方法选用德国非凡SanderⅡ矫治器组件、上海康桥齿科钢丝和上海贝琼自凝胶分别制作SanderⅡ矫治器和肌激动器;病例纳入标准是替牙晚期或恒牙早期,AngleⅡ类Ⅰ分类功能性、齿槽性和骨性患者,深覆牙合深覆盖,均角或底角患者;32例患者来源于西安交通大学医学部口腔医院正畸科(2008-2014年),试验组16例配戴SanderⅡ矫治器,对照组16例配戴肌激动器。治疗前后拍头颅定位侧位片,并用X线头影测量分析法分析治疗前后X线数据,并采用SPSS20.0软件进行统计学分析。
Objective To compare the structure,fabrication principle and clinical efficacy of Sander Ⅱappliance and activator so as to understand the advantages of class Ⅱ division Ⅰ malocclusion of Sander Ⅱappliance.Methods We used materials of German extraordinary Sander Ⅱ appliance assembly,Shanghai Cambridge dental wire and Shanghai Bei Joan form gel to make Sander appliance and activator.The inclusion criteria were late mixed dentition or early permanent dentition;Angle classⅡ divisionⅠ malocclusion;functional,alveolar and bony patients;and deep overbite patients with deep overburden,angle or corner.The cases came from Department of Orthodontics,Stomatology Hospital of Xi'an Jiaotong University between2008 and2014.A total of32 patients were included.In the experimental group 16 patients wore SanderⅡ appliance while another 16 in the control group wore the activator.Lateral cephalometric radiographs were taken before and after treatment,and X cephalometric method was used to analyze the X-ray data before and after treatment.Paired t test statistical analysis was made with SPSS 20.0software.Results The experimental and control groups differed significantly in∠ANB,∠SNA,Wits,∠NA-Pg,∠U1-NA,U1-pp,IMPA,∠L1-NB,L1-NB distance and∠N'-Sn-Pg,suggesting that the maxillomandibular relation improved significantly,deep overbite in deep overburden was normal,the mandible was forward significantly,soft tissues changed obviously,maxillary convexity had a better change,and the lower front teeth were slightly forward.Conclusion SanderⅡ appliance has better therapeutic effects than the activator and the treatment duration can be shortened by 1 year.
