肾肿瘤的超声造影研究

The Characteristics of Contrast-enhanced Ultrasound in Renal Tumors

目的探讨各种不同肾肿瘤的超声造影表现。方法回顾性分析我院超声科138例肾肿瘤,应用超声造影观察肿块回声增强时间和造影剂消退时间,观察肿块的假包膜,本研究的所有138例患者均经过手术和病理证实。结果 138例肾肿瘤中,透明细胞癌和乳头状癌109例,移行细胞癌14例,肾盂鳞癌10例,肾盂上皮细胞癌4例,肾母细胞瘤6例,嗜酸细胞瘤2例,透明细胞癌和乳头状癌109例中有62例出现假包膜,二维声像图显示假包膜41例,造影剂显示快进快出79例,快进慢退14例,慢进慢退11例,肿块局部回声增强5例。结论超声造影有助于鉴别肾肿瘤,有助于显示肾细胞癌的假包膜。

Objective To investigate the characteristics of contrast—enhanced ultrasound in renal tumors.MethodsOne hundred and thirty eight patients with renal tumors were prospectively studied with CEUS,when using Sono Vue to examine the renal tumors,the time of tumors enhancement and wash-out of the contrast were observed and pseudocapsule in renal cel carcinomas were detected,al patients with renal tumors had proven by operation and histological findings.Results One handred and nine solid renal cel carcinomas,fourteen transitional cel carcinomas of renal pelvis,ten squamous cel carcinomas,four epidermoid carcinoma,six whilms tumors and leukocyte cel tumors were analyzed,sixty-two RCCs showed pseudocapsule with CEUS compared to fourty one two at baseline sonography,sevevty-nine RCCs revealed a high wash-in and washout with CEUS,fourteen RCCs showed a high wash-in and the delayed wash-out with CEUS,the delayed wash-in and wash-out were observed in eleven RCCs,five RCCs showed focal enhancement with CEUS,RCCs revealed a higher contrast enhancement than angiomyolipomas with CEUS.Conclusion CEUS may differentiate renal tumors and show clearly pseudocapsule in RCCs.
