482例骨质疏松性骨折住院患者再次骨折分析

Refracture analysis of 482 cases of osteoporotic fracture in hospitalized patients

目的探讨骨质疏松性骨折（osteoporosis-related fractures）的再次骨折发生率、出院后是否坚持服药与再次骨折的关系,强调坚持服药的重要性,进一步指导临床诊治工作。方法回顾性分析2010年1月-2014年8月某医院收治的482例骨质疏松性骨折患者的临床资料,电话随访患者出院后情况,将再次骨折的患者根据是否坚持抗骨质疏松治疗分成两组进行对比,对比其再次骨折发生率及两次骨折间隔时间长短的差异,说明骨质疏松预防用药的重要性。结果 1再次骨折率为11.2%,其中第一次骨折出院后坚持服药仍骨折的患者占5.5%,未坚持服药发生骨折的患者占12.9%,（P=0.032）。2第一次骨折至第二次骨折之间平均间隔时间为23.89个月,其中第一次骨折后坚持服药的患者发生第二次骨折的间隔时间为36.50±1.64月,第一次骨折后不坚持服药的患者发生第二次骨折的间隔时间为22.31±4.60月,（P=0.000）。结论出院后坚持服用抗骨质疏松药物对预防再次骨折有一定的作用,第一次骨折后坚持抗骨质疏松治疗的患者发生再次骨折的间隔时间比不坚持抗骨质疏松治疗的患者发生再次骨折的间隔时间要长。抗骨质疏松治疗可减少再次骨折的发生。

Objectives To detect the incidence of re-fracture of osteoporosis-related fractures,wheher or not adhere to medication and the relationship between refracture after discharge,to emphasize the importance of adherence to medication and to further guide clinical diagnosis and treatment. Methods Retrospective analysis the clinical data of 482 cases of patients with osteoporotic fractures from January 2010 to August 2014 in a hospital. Follow-up by telephone the situation after discharge,the patients re-fractured are divided into two groups to compare according to whether or not to adhere to anti-osteoporosis treatment. Compare include the incidence of re-fracture and time interval between two fractures,illustrate the importance of preventive medication of osteoporosis.Results 1Re-fracture rate of 11. 2% after discharge,patients who adhere to the medication re-fracture accounted for 5. 5%,patients who not adhere to medication re-fracture accounted for 12. 9%（ P = 0. 032）. 2The average interval between the first fracture and the re-fracture was 23. 89 months,fracture patients adhere to medication re-fracture interval was 36. 50 ± 1. 64 months,fracture patients not adhere to medication re-fracture interval was 22. 31 ± 4. 60 months（ P = 0. 000）. Conclusions Insist on taking the anti-osteoporosis drugs on the prevention of re-fracture to a certain extent after discharge. The time interval between two fractures that patients who adhere to anti-osteoporosis treatment after first fracture is longer than the interval that patients who do not adhere to antiosteoporosis treatment. Anti-osteoporosis treatment can reduce the re-occurrence of fractures.
