1，3-β-D葡聚糖在侵袭性真菌感染临床诊断中的价值研究

目的探讨血浆1,3-β-D葡聚糖检测在侵袭性真菌感染临床诊断中特异性和敏感性及其价值。方法2013年10月至2014年10月我院收治的70例侵袭性真菌感染患者作为观察组，选取我院同期收治的70例非侵袭性真菌感染患者作为对照组，比较观察组与对照组血浆1，3-β-D葡聚糖检测结果，并分析血浆1,3-β-D葡聚糖检测的敏感性、特异性和准确度。结果观察组以白色假丝酵母菌（28．57％）、卡氏肺孢子菌为主（24．28％），两组菌种分类比较差异有统计学意义（P〈0．05）。观察组组患者血清1，3-β-D葡聚糖浓度显著高于对照组，两组比较差异有统计学意义（P〈0．05）。1,3-β-D葡聚糖在侵袭性真菌感染临床诊断的敏感性为89．83％，特异性为81．81％，准确性为88．57％。结论1,3-β-D葡聚糖检测在侵袭性真菌感染早期诊断中敏感性和特异性均较好，尤其适用于白色假丝酵母菌和菌烟曲霉菌等引起的侵袭性真菌感染，临床应用价值较高。

Objective To study the diagnostivalue, specificity and sensitivity of 1,3-β-D-glucan in invasive fungal infection. Method70 caseof invasive fungal infection in ouhospital from Octobe2013 to Octobe2014 were selected aobservation group, 70 caseof noninvasive ftmgal infection were selected acontrol group. Compared the level of 1,3-β-D-glucan of two groups. Analyzed sensitivity, specificity and accuracy of 1,3-β-D-glucan detection. ResultThe majofunguspeciewere Candidalbican（28.57%）, Pneumocysticarinii （24.28%）, there wastatistically significandifference in funguspecieclassification between two group（P〈0.05）. The level of 1,3-β-D-glucan in observation group wasignificantly highethan thain control group, with statistically significandifference （P〈0.05）. The sensitivity of 1,3-β-D-glucan in the clinical diagnosiof invasive fungal infection wa89.83%, the specificity wa81.81% and the accuracy wa88.57%. Conclusion 1,3-β-D-glucan in the early diagnosiof invasive fungal infection habettesensitivity and specificity, especially suitable foinvasive fungal infection caused by Candidalbicanand aspergillus, with high clinical value.
