中药联合自体表皮移植治疗白癜风的临床研究

Clinical Study on Treatment of Vitiligo by the Autologous Epidermal Graft Combination with Chinese Medicine

目的探讨自体表皮移植联合中药治疗白癜风的临床疗效及作用机制。方法采用酶联免疫吸附试验(ELISA)法检测白癜风患者单纯自体表皮移植和同时联合中药治疗前后局部皮损组织液中白细胞介素(IL)-18、肿瘤坏死因子(TNF)-α以及可溶性细胞间黏附分子(s ICAM-1)水平,并进行疗效评价。结果治疗前白癜风患者局部皮损组织液中IL-18、TNF-α以及s ICAM-1水平明显高于健康对照组(P<0.01),联合治疗组治疗后各细胞因子水平基本恢复正常,且联合治疗组显效率明显高于单纯移植组(P<0.05)。结论白癜风患者局部皮损处存在免疫状态异常,中药可以通过调节机体免疫功能提高表皮移植的成功率。

Objective To observe the clinical effects and mechanism of the autologous epidermal graft combination with Chinese medicine in treating vitiligo. Methods ELISA was used to detect the levels of IL-18, TNF-α, sICAM-1 in skin tissue fluid before and after autologous epidermal graft only or it combination with Chinese medicine. And the curative effects were evaluated. Results Before treatment, the level of IL-18, TNF-α, sICAM-1 in skin tissue fluid from patients was significantly higher than those from healthy controls（P〈0.01 ）. Each cytokine levels in combination group after treatment basically returned to normal, and the significant effective rate of combined treatment group was obviously higher than that of pure transplantation group （P〈0.05）. Conclusion Vitiligo patients exists abnormal immune status in skin lesions, and the Chinese medicine can improve the success rate of the autologous epidermal graft by regulating the body＇s immune function.
