置管引流术结合虚挂线治疗高位肛瘘临床研究

Clinical study of catheter drainage combined with loose thread-drawing in treating high anal fistula

目的观察置管引流术结合虚挂线治疗高位肛瘘的临床疗效。方法将68例高位肛瘘患者随机分为治疗组和对照组,每组34例;治疗组予置管引流术结合虚挂线治疗,对照组予低位切开高位挂线治疗;观察两组治愈率、创口愈合时间、复发率、术后疼痛评分、肛管直肠压力及肛门功能情况。结果治疗组和对照组治愈率分别为94.12%和82.35%,治疗组高于对照组(P<0.05);治疗组创口愈合时间少于对照组(P<0.05);术后第7天和第10天时,治疗组疼痛积分低于对照组(P<0.05)。结论置管引流术结合虚挂线治疗高位肛瘘可在有效保护肛门功能的基础上提高疗效,降低术后并发症。

Objective To observe the clinical effect of catheter drainage combined with loose thread-drawing on high anal fistula. Methods 68 cases of high anal fistula patients were randomly divided into treatment group and control group, 34 cases in each group. Patients in treatment group were treated with catheter drainage combined with loose thread-drawing; patients in control group were treated with inferior incision combined with tight thread-drawing. The curative rate, wound healing time, recurrence rate, postoperative pain, anoreetal pressure changes to evaluate anal function were compared between the two groups. Results The cure rate of treatment group and control group were 94.19% and 82.35% respectively, the treatment group was higher than control group （ P 〈 0.05 ）; in wound healing time, the treatment group was shorter than the control group （P 〈 0.05 ） ; in postoperative pain after surgery 7 days and 10 days, the treatment group pain score was less than the control group （ P 〈 0.05 ）. Conclusion Catheter drainage combined with loose thread-drawing on high anal fistula can be increased on the cure rate and reduce postoperative pain on the basis of the effective protection of anal function.
