牵引对腰椎间盘DTI扩散能力的影响及意义

摘要 目的:观察腰椎间盘突出症患者行牵引治疗前后磁共振扩散张量成像(DTI)的变化,探讨DTI在椎间盘病变诊断及治疗中的应用价值。方法:60例腰椎间盘突出症患者行规范腰椎多功能牵引床牵引治疗3周。分别记录治疗前后椎间盘DTI评价参数ADC、FA值的变化,使用疼痛视觉模拟量表(VAS)评分和日本骨科学会(JOA)腰腿痛评分评价疗效。结果:患者治疗后的ADC值、JOA评分明显高于治疗前,FA值、VAS评分明显低于治疗前(P<0.01)。结论:DTI是腰椎间盘病情变化及治疗效果的一个良好评价指标,牵引治疗能影响腰椎间盘的DTI扩散能力。
