进展性脑梗死危险因素的临床研究

Clinical study of progressive cerebral infarction risk factors

目的研究进展性脑梗死的危险因素。方法选取68例进展性脑梗死患者（观察组）与72例完全性脑梗死患者（对照组），观察两组患者不同数据出现频率，寻找进展性脑梗死的危险因素，对危险因素进行组间比较，发现最多出现的因素，同时对危险因素中的具体数值进行比较。结果与对照组比较，观察组22种可能危险因素中9项因素有差异，其中发病时随机血糖、白细胞计数、治疗期间血压降低过快和纤维蛋白原显著升高（P〈0．05）；观察组脑血管粥样硬化软斑块形成、感染合并发热、焦虑症、酸碱失衡和水电解质紊乱明显高于对照组（P〈0．001）；9种因素中以脑血管粥样硬化软斑块形成46例（67．64％）、治疗期间血压降低过快44例（64．70％）、随机血糖增高46例（67．64％）、感染合并发热35例（51．47％），同时发现多种因素合并存在率越多进展性脑梗死的发生可能性越大。结论进展性脑梗死的发生可能是综合因素和多种复杂机制共同作用的结果。

Objective To study the risk factors of progressive cerebral infaration. Methods 68 patients with stroke in progression （observation group） and 72 patients with complete cerebral infarction （control group） were studied. The rates of occurrence of the patients of the two groups were evaluated to explore the related factors of progressive cerebral infarction. The most factors were discovered by comparing the risk factors in the two groups. Results Compared with the control group, 9 of the 22 possible risk factors were different in the observation group. The GLU, the number of leucocyte, the dropping speed of the blood pressure during the treatment, FIB in the observation group were higher than the control group （P〈0.05）. The formation of cerebral atherosclerosis plaques, infection with fever, anxiety disorder, acid-base disorder, water and electrolytes disorder in the observation group were higher than the control group. The formation of cerebral atherosclerosis plaque in the 9 factors was 46 （67.64%）. The speed of dropping of the blood pressure during the treatment was 44 （64.70%）. The increase of the GLU 46 （67.64%）. The infection with fever was 35 （51.47%） as compared with the other factors （P〈0.05）. The more various factors combined, the more likely that occurrence of progressive cerebral infarction have. Conclusion The occurrence of progressive cerebral infarction are related to many factors.
