摘要
目的研究侧裂区低级别脑胶质瘤中基质金属蛋白酶(MMP)家族水平的检测及其与预后情况的相关性。方法将2007年1月至2010年12月四川大学华西医院收治的130例侧裂区Ⅰ、Ⅱ级胶质瘤患者作为观察组,选择同期收治的30例颅脑损伤患者作为对照组,检测组织中MMP-2、MMP-9的mRNA和蛋白水平,并随访观察组患者的预后情况。结果观察组患者肿瘤组织中MMP-2、MMP-9的mRNA水平分别为233±35、253±32,均高于对照组的100±15、100±19,差异有统计学意义(P<0.05);观察组患者肿瘤组织中MMP-2、MMP-9的蛋白阳性率分别为63.8%(83/130)、62.3%(81/130),均高于对照组的13.3%(4/30)、16.7%(5/30),差异有统计学意义(P<0.05)。MMP-2、MMP-9阳性患者的无瘤生存时间短于阴性患者,1年复发率、3年复发率高于阴性患者(P<0.05);MMP-2、MMP-9是胶质瘤患者1年复发(OR=2.275,2.311)、3年复发(OR=2.291,2.541)的危险因素。MMP-2、MMP-9阳性患者的无瘤生存时间短于阴性患者(P<0.05),1年复发率、3年复发率高于阴性患者(P<0.05);MMP-2、MMP-9是胶质瘤患者1年复发、3年复发的危险因素。结论 MMP-2、MMP-9的表达与侧裂区脑胶质瘤患者的预后密切相关,MMP-2、MMP-9阳性患者的远期复发率较高。
Objective To study the levels of matrix metalloproteinase (MMP) family in lateral fissure area low grade glioma and its correlation with prognosis. Methods Total of 130 patients with lateral fissure area grade Ⅰ and Ⅱ glioma in West China Hospital of Sichuan University from Jan. 2007 to Dec. 2010 were enrolled in observation group and another 30 brain injury patients in the same period were enrolled in control group. Then mRNA and protein levels of MMP-2, MMP-9 were detected, and the prognosis were followed up. Results mRNA levels of MMP-2, MMP-9 in tumor tissue of the observation group were 233 ± 35 and 253 ± 32, higher than the control group( 100 ± 15 and 100 ± 19 ) ,the differences were statistically significant (P 〈 0.05). The positive rate of MMP-2, MMP-9 protein in tumor tissue of the observation group were 63.8% (83/130) and 62.3% (81/130) ,which were higher than the control group[ 13.3% (4/30) and 16.7% (5/ 30) ] ,the differences were statistically significant ( P 〈 0.05 ). Tumor free survival time of MMP-2, MMP-9 positive patients was shorter than negative patients, and the 1-year and 3-year recurrence rate were higher than. the negative patients ( P 〈 0.05 ). MMP-2 and MMP-9 were risk factors of 1-year recurrence ( OR = 2. 275,2. 311 ) and 3-year recurrence ( OR = 2.291,2. 541 ). Conclusion MMP-2 and MMP-9 expression are closely correlated with prognosis of lateral fissure area glioma patients;long-term recurrence rate of MMP-2, MMP-9 positive patients is higher.
