某院Ⅰ类切口围手术期抗菌药物的预防应用调查研究

Analysis on the usage of the prophylactic antibiotics with typeⅠincision operation

目的调查某院2015年1∽6月Ⅰ类切口围手术期预防使用抗菌药物的情况,并分析研究其用药的合理性。方法收集某院2015年1∽6月所有科室进行Ⅰ类切口手术的病史资料2 036例,调查其预防使用抗菌药物的情况。结果 2 036例病例中,抗菌药物预防使用率为40.52%（825例）。在825例预防性用药中,术前给药时机控制在0.5∽2 h以外的为195例（23.64%）;预防用药时间超过24 h的为368例（44.61%）;抗菌药物选择不适宜的为262例（31.75%）。结论该院在Ⅰ类切口围手术期预防使用抗菌药物问题上存在不合理性,主要集中在预防使用抗菌药物指征把握不严,抗菌药物选择不适宜以及术后使用时间长等问题上,亟需采取相应措施加以干预。

Objective To survey the usage of prophylactic antimicrobial agents in patients with type I incision operation in a hospital from January 2015 to June 2015. Methods 2 036 patients with type I incision operation who discharged from hospital from January to December 2015 were investigated and analysed. Results Among all the 2 036 cases,825cases were used for prophylaxis,the usage rate was 40.52%. There are 95 cases（11.52%）received antibiotics in the according to the prescribed requirements preoperative 0.5-2.0 hours prophylaxis. There are 268 cases（32.48%）who preventive medication for more than 48 hours;There are 462 cases（56.00%）who antimicrobial drug selection is not appropriate. Conclusion There are problems in prophylactic antimicrobial usage in patients with type I incision operation in this hospital,the indications are not stringent,category of the antimicrobial agents is not chosenproperly;the using occasion is not appropriate,antimicrobial agents is excessively used. It is urgent to strengthenthe management of standard antimicrobial usage.
