冠状动脉慢性完全闭塞患者血小板计数相关影响因素 被引量:5

Related influencing factors for blood platelet count with chronic total occlusion of coronary artery

摘要 目的探讨冠状动脉慢性完全闭塞(chronic total occlusion,CTO)患者血小板计数变化及相关影响因素。方法经冠状动脉造影确诊冠心病患者176例,至少有1支主要冠状动脉完全闭塞病变者88例为CTO闭塞组,无冠状动脉闭塞病变者88例为无CTO闭塞组。测定并比较2组入院时血小板计数、血小板平均体积(platelet volume,MPV)、血小板分布宽度(platelet distribution width,PDW)、大血小板比例及高敏C反应蛋白(high sensitivity C-reactive protein,hs-CRP)等指标。结果 CTO闭塞组血小板计数[(202.61±57.02)×109/L]低于无CTO闭塞组[(222.85±54.60)×109/L],hs-CRP[(8.12±5.36)mg/L]高于无CTO闭塞组[(4.08±2.66)mg/L],2组比较差异有统计学意义(P<0.05);2组MPV、PDW、大血小板比例等比较差异无统计学意义(P>0.05);Pearson相关分析结果显示,血小板计数与hs-CRP呈负相关(r=-0.705,P=0.027)。结论 CTO患者血小板计数减少与hs-CRP升高相关,血小板计数和hs-CRP是用于CTO患者病情观察和评价的有效指标之一。 Objective To explore the change of blood platelet count (BPC) in patients with chronic total occlusion (CTO) of coronary artery and its related influencing factors. Methods A total of 176 patients with coronary angiographically confirmed coronary heart disease were divided into CTO group (n= 88) with CTO in at least one main coronary artery, and non-CTO group (n = 88) with no CTO in coronary artery. Their BPC, mean platelet volume (MPV), platelet distribution width (PDW), platelet large cell ratio (P-LCR) and high sensitivity C-reactive protein (hs-CRP) at admission were detected and compared between two groups. Results BPC was lower and hs-CRP was higher inCTO group ((202. 61± 57. 02)×10^9/L, (8. 12 ± 5. 36) mg/L) than those in non-CTO group ((222. 85± 54.60)×10^9/L, (4.08±2. 66) mg/L) (P〈0.05). There were no significant differences in MPV, PDW and P-LCR between two groups (P〉0.05). Pearson correlation analysis showed that BPC was negatively correlated with hs-CRP (r=-0. 705, P=0. 027). Conclusion Decreased BPC is correlated with the increased hs-CRP in CTO patients. BPC and hs-CRP are the useful indicators for observing and evaluating CTO.
