近日节律对靶控输注丙泊酚BIS值影响研究

摘要 目的探讨近日节律对靶控输注丙泊酚时脑电双频指数(BIS)值的影响。方法选择全身麻醉下手术患者60例,分为日间组(D组,10:01-20:00)与夜间组(N组,20:01-10:00),每组各30例,丙泊酚采用靶控输注法,血浆靶控浓度设定为3μg/ml,持续记录患者ECG、MAP、HR、SPO 2及BIS值。结果两组患者意识消失时MAP、SPO 2、HR比较无统计学差异,夜间组基础BIS值偏低,当OAA/S评分为2分及1分时,夜间组BIS值低于日间组(P<0.05)。结论近日节律对靶控输注丙泊酚BIS值有影响,表现为夜间BIS值较日间BIS值偏低。 Objective To investigate the effect of circadian rhythm on bispectral index(BIS) for propofol by TCI. Methods 60 patients scheduled for surgeries under general anesthesia were divided into 2 groups(n=30 each): day- time group(D group,from 10:01 to20:00)and night- timegroup(N group, from20:01 to10:00). Propofol wasinfused by TCI, thetarget plasmaconcentration of propofol wasset at 3.0μg/ml. ECG、MAP、HR SPO2 and BIS were recorded. Results TThere was no significant difference in MAP、HR and SPO2 between the two groups when consciousness lost,the baseline BIS value was lower in group N than group D.The BIS value was lower in group N than group D when OAA/Sscore=2 and OAA/Sscore=1(P 0.05). Conclusion Circadian rhythm has influence on BISvalue for propofol by TCI, the BISvaluewaslower in night than duringtheday time.
