Analysis on irrational medication orders of Pharmacy Intravenous Admixture Service in Central Hospital of Zibo from November 2012 to May 2015

目的对淄博市中心医院静脉用药调配科(PIVAS)不合理医嘱的情况进行分析,为临床合理用药提供参考。方法调取淄博市中心医院PIVAS 2012年11月—2015年5月静脉用药医嘱985 982条,对其中不合理医嘱的药物使用情况进行评价和分析。结果共涉及不合理医嘱2 018条,约占总医嘱的0.20%,主要包括配伍不当、给药频次或间隔不当、给药剂量不当或溶媒剂量不合理和溶媒选择不当。结论对静脉用药医嘱的审核和干预可以促进合理用药,审方药师积极与临床医师沟通,在药学服务中发挥了积极的作用。

Objective To investigate the irrational medication orders of Pharmacy Intravenous Admixture Service（PIVAS） in Central Hospital of Zibo, and to provide reference for rational drug use. Methods Intravenous medication orders（985 982 cases） in Central Hospital of Zibo from November 2012 to May 2015 were checked, and the irrational medication orders were evaluated and analyzed. Results 2 018 Cases of medication orders were irrational, and accounted for 0.20% of the total, which mainly included improper compatibility, irrational medication frequency, inappropriate administration dosages or unreasonable solvent dosages, inappropriate dosages or concentrations, and inappropriate selection of solvent. Conclusion The periodic check and intervention on the medication orders can promote the clinical rational medication. Active communicating between prescription-checking pharmacists and clinicians has played a positive role in the pharmaceutical service.
