术后早期活动计划在腹腔镜胆总管术后患者中的应用

Application of early postoperative activity plan in patients with laparoscopic common bile duct surgery

目的 :探讨术后早期活动计划在腹腔镜胆总管术后的应用效果。方法 :选取肝胆外科腹腔镜胆总管术后患者185例,按照护理方法的不同,分为观察组93例和对照组92例。对照组采用常规护理,观察组严格按照术后早期活动计划进行护理,观察两组患者的术后首次下床活动时间、术后排气时间、住院天数、深静脉血栓(DVT)及肺部感染发生率,评价患者满意度。结果 :观察组术后首次下床活动时间短于对照组(P<0.05),患者术后排气时间、住院天数、DVT及肺部感染发生率、满意度明显优于对照组(P<0.05)。观察组伤口疼痛、担心伤口裂开、管道脱出及心理方面情况要少于对照组(P<0.05)。结论 :术后早期活动计划能够促进腹腔镜胆总管患者术后早期下床活动,从而提早患者术后排气时间,减少住院天数,预防术后并发症的发生,提高患者的满意度。

Objective： To explore the effects of early postoperative activity plan in early postoperative ambulation of patients with endoscopic common bile duct surgery. Methods： Totally 185 cases of patients after laparoscopic common bile duct operation were selected in this study. They were divided into two groups, the observational group（93 cases） and the control group（92 cases）. Patients in the control group received usual care, while patients in observational group got strictly planned care based on the early postoperative activity plan. Time for the first ambulation, postoperative exhausts time, hospitalization days, incidence of DVT and pulmonary infection, and patients＇ satisfaction were collected and analyzed. Results： Time for the first ambulation in the study group was shorter than that in the control group（P〈0.05）. Postoperative exhausts time, hospitalization days, incidence of DVT and pulmonary infection, and patients＇ satisfaction were better in the study group than those in the control group（P〈0.05）. The rates of pain complaints, worry about wound dehiscence, unexpected extubation, and mental problems were less than control group（P〈0.05）. Conclusion： Early postoperative activity plan can encourage early postoperative ambulation in patients with endoscopic CBD surgery, and then promote postoperative exhaust, shorten hospital stay, reduce postoperative complications, and improve patients＇ satisfaction.
