摘要
目的:探讨正常及退变软骨磁共振T1驰豫值及T2弛豫值的定量研究价值。方法:采用PHILIPS Acheiva 3.0超导型磁共振仪进行T1弛豫值及T2弛豫值的定量测定,选择60例正常及退变膝关节,其中正常组(n=20)、OA(退行性骨关节炎,osteoarthritis)组(n=40);OA组按照kellgren-Lawrence评分又分为轻度OA组(n=20)、中度OA组(n=11)及重度0A组(n=9)。结果:正常组、轻度OA组、中度OA组、重度OA组平均T2驰豫值分别为(39.31±3.9)ms、(48.58±8.53)ms、(54.92±8.84)ms、(59.03±6.30)ms;采用协方差分析,协变量年龄对T2值有明显的影响(F=5.745,P=0.018<0.05);四组髌软骨T2值有明显差异(F=26.579,p=0.00<0.05)。正常组、轻度OA组、中度OA组、重度OA组平均T1弛豫值分别为(511.70±72.25)ms、(458.03±55.89)ms、(405.89±57.30)ms、(357.92±36.74)ms。采用单因素方差分析,四组T1弛豫值有明显的差异(F=17.70,P=0.00<0.05)。结论:T2弛豫值、T1弛豫值随着退变程度的加重发生明显变化,其定量分析在早期OA诊断中有较高的临床应用价值。
Purpose: To evaluate the diagnostic value of T1 relaxation time and T2 relaxation time in patella cartilage of asymptomatic volunteers and osteoarthritis (OA). Methods: Quantitative T1 relaxation time and T2 relaxation time of patella cartilage from 60 people were evaluated on a 3.0T MR scanner. The study population was categorized as normal group (n-20) and OA group (n=40). And the OA group was stratified by kellgren- Lawrence into 3 cohorts: mild OA group (n=20), moderate OA group (n=ll) and severity OA group (n=9). Results: The average T2 relaxation time of patella cartilage in normal, mild OA, moderate OA and severity OA group were (39.31±3.9) ms, (48.58±8.53) ms, (54.92±8.84) ms, (59.03±6.30) ms. respectively; there were significant differences of average T2 relaxation time among these four groups (F=26.579, P=0.00〈0.05) and age had an effect on T2 relaxation (F=5.745, P=0.018〈0.05) by covariance analysis. The average T1 relaxation of patella cartilage in normal group, mild OA, moderate OA and severity OA group were (511.70±72.25) ms, (458.03±55.89) ms, (405.89±57.30) ms, (357.92±36.74) ms, respectively; there were significant differences of average T1 relaxation time among these four groups (F=17.70, P=0.00〈0.05). Conclusion: The study of T2 relaxation time and T1 relaxation time of patella cartilage could indicate component abnormality of articular cartilage at the degree of early OA, and had a high value in early OA diagnosis.
