摘要
目的探讨我国中青年男性腹部脂肪分布特征。方法采用定量CT测量62名男性脐上11cm至脐下8cm共20个层面总脂肪面积(TFA)、腹腔内脂肪面积(VFA)、皮下脂肪面积(SFA),并计算总腹部脂肪体积(TTFV)、总腹腔内脂肪体积(TVFV)、总腹部皮下脂肪体积(TSFV)。以BMI 25kg/m2为界,分为BMI〈25kg/m2(非超重/肥胖)组和BMI≥25kg/m2(超重/肥胖)组,各31名,并进行统计分析。结果 VFA从下腹部至上腹部逐渐增加。BMI≥25kg/m2组和BMI〈25kg/m2组最大SFA分别位于脐下4cm和2cm,随上、下层面的增加SFA逐渐减小。所有层面TFA、VFA、SFA均与相应的TTFV(R2=0.87~0.98)、TVFV(R2=0.77~0.97)、TSFV(R2=0.84~0.96)相关。VFA在脐上4cm、5cm层面与TVFV相关性较其他层面更显著(R2=0.98、0.97)。TVFV与WC、WHR、BMI、TSFV、TTFV有相关性,与臀围(HC)无相关性。结论我国中青年男性腹部脂肪分布具有特征性;VFA在脐上4cm、5cm最能代表TVFV;WC、WHR、BMI、TTFV、TSFV与TVFV的相关性均弱于腹部任意层面VFA。
Objective To investigate the distribution of abdominal fat in Chinese middle-aged young men. Methods A total of 34 male subjects were enrolled in this study.All the subjects were divided into two groups according to their BMI:overweight/obesity group(BMI≥25kg/m^2)and non overweight/obesity group(BMI25kg/m^2).With navel as the locating landmark,area from 11 cm above to 8cm below was scanned by QCT to obtain 20 planes at 11 cm interval.Total fat area(TFA),visceral fat area(VFA)and subcutaneous fat area(SFA)were measured and total abdominal fat volume(TTFV),total visceral fat volume(TVFV)and total subcutaneous fat volume(TSFV)were calculated. Results VFA gradually increased from lower abdomen to upper abdomen.The highest SFA was at 2cm and 4cm below navel for non overweight/obesity group and overweight/obesity group respectively.The SFA gradually decreased with interval increased upward and downward.TFA,VFA and SFA derived from each plane were significantly correlated with corresponding TTFV(R2=0.87~0.98),TVFV(R2=0.77~0.97)and TSFV(R2=0.84~0.96).VFA measured at 4cm or 5cm above navel had more significant correlation with TVFV as compared with other plane(R2=0.98、0.97).TVFV was associated with WC,WHR,BMI,TSFV and TFV,except HC. Conclusion The distribution of abdominal fat in Chinese middle-aged young men has special characteristics.VFA at 4cm or 5cm above navel has the most significant association with TVFV.TVFV has a more significant association with VFA than with WC,WHR,BMI,TTFV and TSFV at any plane.
