经纤维支气管镜支气管肺泡灌洗抗真菌药治疗肺部真菌感染临床观察

Clinical observation of bronchoalveolar lavage antifungal medicine by fiberbronchoscopy in treatment of pulmonary fungal infection

目的 观察经纤维支气管镜支气管肺泡灌洗抗真菌药物治疗肺部真菌感染的临床疗效.方法 选择78例肺部真菌感染患者作为研究对象,采用随机对照表将患者分为对照组和观察组,每组39例,对照组行常规治疗,观察组在对照组基础上行经纤维支气管镜支气管肺泡灌洗抗真菌药治疗,记录两组体温恢复、肺啰音减少、白细胞计数恢复、痰液培养转阴时间,并评估临床疗效.结果 观察组治疗有效率为91.67％,显著高于对照组（72.22％）,差异有统计学意义（P＜0.05）.观察组体温恢复、肺啰音减少、白细胞计数恢复、痰液培养转阴时间分别为（4.23±0.94）d、（5.72±1.16）d、（4.86±1.30）d、（5.33±1.41）d,显著低于对照组,差异有统计学意义（P＜0.05）.结论 在常规治疗基础上辅以经纤维支气管镜支气管肺泡灌洗抗真菌药治疗肺部真菌感染,能够缓解肺部炎症,提高临床治疗效果.

Objective To observe the effect of bronchoalveolar lavage antifungal drug by fiberbronchoscopy on pulmonary fungal infection.Methods Seventy-eight cases of pulmonary fungal infection were chosen as research objects,and were randomly divided into control group （39 cases） and observation group （39 cases）.The patients in control group received routine treatment,and the patients in observation group received bronchoscopic bronchoalveolar lavage antifungal therapy on the basis of treatment in the control group.The body temperature recovery,pulmonary rales decreased,white cell counts recover,sputum culture negative conversion time was recorded,and the clinical efficacy was evaluated.Results The effective rate of the treatment group（91.67%） was significantly higher than that of the control group（72.22%）,there was significant difference （P 〈 0.05）.In observation group,the temperature recovery time,lung rale decreasing time,white cell counts recovery time and sputum negative time was （4.23 ±0.94） d,（5.72 ± 1.16） d,（4.86 ± 1.30） d,（5.33 ± 1.41） d,which was significantly lower than those of the control group（P 〈 0.05）.Conclusions On the basis of routine treatment,combined with fiberbronchoscopic bronchoalveolar lavage antifungal agents for the treatment of pulmonary fungal infection,can alleviate the lung inflammation,and improve the clinical treatment effect.
