局部晚期喉癌容积弧形调强放疗与固定野调强放疗的剂量学比较

摘要 目的比较容积弧形调强放疗(volumetric modulated arc radiotherapy,VMAT)与固定野调强放疗(intensity-modulated radiotherapy,IMRT)技术在局部晚期喉癌应用中的剂量学差异。方法对11例局部晚期喉癌患者分别制定双弧VMAT与七野IMRT两组放疗计划。比较剂量体积参数,适形指数(CI),均匀指数(HI),计划设计时间,机器跳数(monitor unit,MU)和投照时间。结果高危靶区PTV1:VMAT的CI和HI明显优于IMRT(P<0.05)。低危靶区PTV2:VMAT的CI优于IMRT(P<0.05),HI与IMRT相当(P>0.05)。VMAT计划的脊髓(计划区)和脑干(计划区)的Dmax以及右侧腮腺的Dmean、V30Gy均低于IMRT(P<0.05),其他器官剂量两者相似(P>0.05)。VMAT计划设计时间较IMRT增多213%(P<0.05),MU与投照时间分别较IMRT减少64%与66%(P<0.05)。结论VMAT能达到较优的靶区剂量分布,保护部分危及器官,大幅减少MU及投照时间。
