摘要
目的针对新购进ADVIA 2120i血液分析仪报警信息进行可靠性分析。方法选取500例有异常报警信息提示的和500例无报警提示的门诊和住院患者的血液标本,进行手工涂片,经瑞姬氏染色,显微镜镜检复核对比。结果对于有报警信息提示的标本中未成熟粒细胞(IG)、血红蛋白浓度不等(Hc-VAR)、小红细胞(MICRO)、核左移(Ls)、大红细胞(MACRO)、低色素性红细胞(HYPO)、高色素性红细胞(HYPER)、红细胞大小不等(ANISO)仪器报警信息与显微镜镜检符合率差异无统计学意义(P>0.05);而有核红细胞(NRBC)差异有统计学意义(P<0.05);大血小板(LPLT)、原幼细胞(BLASTS)、变异淋巴细胞(ATYP)、血小板凝集(PLTCLM报警信息两者之间比较,差异有统计学意义(P<0.01)。无报警信息的标本中MACRO、MICRO、HYPO、ANISO、LPLT、PLTCLM报警的标本有少量漏检。结论仪器对于有核左移、红细胞异常方面的报警信息的和无报警信息的标本能起到良好的过筛作用。而对于有血小板、幼稚细胞特别是早期的幼稚细胞、异型淋巴细胞方面的报警信息的标本,应涂片进行显微镜手工复捡。
Objective To evaluate the reliability of the alarm system of the newly - purchased AD- VIA 2120i hematology analyzer. Methods Five hundred blood specimens with the information tips of abnor- mal alarm and 500 cases of blood specimens without alarm tips were collected from the outpatients and inpa- tients. After manual smearing,via Wright's staining, the review comparison was done under microscope. Re- suits Regarding the coincidence rate, among the specimens with alarm information tips, the results of imma- ture granulocyte ( IG), hemoglobin concentration variation ( Hc - VAR ), microcyte ( MICRO ), nuclear left shift ( Ls), macrocyte ( MACRO ), hypochromic erythrocyte ( HYPO ), hyperchromic erythrocyte ( HYPER ) and anisocytosis (ANISO)were not different significantly as compared with those reviewed under microscope( P 〉 0.05 ). But, the difference was significant in the results of nuclear red blood cell( NRBC ) ( P 〈 0. 05 ) , plate- let( LPLT), blasts ( BLASTS ), atypical lymphocyte ( ATYP ) and platelet clumps ( PLTCLM ) ( P 〈 0.01 ) be- tween the two groups. There was little missed - review in MACRO, MICRO, HYPO, ANISO, LPLT and PLT- CLM among the specimens without alarm information. Conclusion The analyzer has the good screening effect in terms of nuclear left shift and erythrocyte abnormality between the alarm information and non - alarm information. For the specimens with alarm information in the platelets and blasts, especially the early juvenile cells and atypical lymphocytes, the manual smearing review is required under microscope.
