顺行法胃大部切除在胃癌手术中的应用效果观察

The Effect Observation of Antegrade Approach Gastrectomy in Gastric Surgery

目的分析研究顺行法胃大部切除在胃癌手术中的应用效果。方法随机选取2012年4月至2014年5月于本院就诊的60例胃癌患者作为研究对象,所有患者均进行顺行法胃大部切除术,对本组患者的治疗效果以及术后不良反应发生情况进行总结。结果本组60例患者中34例行根治性胃切除术,23例行姑息性胃切除术,3例患者行胰十二指肠切除术,平均清扫淋巴结(23.4±1.4)枚;除1例术后因胰漏死亡外,所有患者经手术治疗后临床症状均有明显好转;本次研究中共6例患者出现术后不良反应(10.0%),其中3例患者切口感染、2例患者肺部感染、1例患者吻合口狭窄、1例患者腹腔出血。结论顺行法胃大部切除术治疗胃癌具有一定的临床疗效,其可在一定程度上降低手术对病变的刺激,且符合外科无瘤手术操作以及无触摸胃癌根治术的基本原则,故具有临床推广应用价值。

Objective To analyze the method of anterograde stomach mostly application effect in gastric cancer surgery. Methods Randomly selected from April 2012 to May 2014 in 60 cases of gastric cancer patients in our hospital as the research object, all patients were suitable method most of gastric resection, the treatment effect of the patients and summarize the postoperative adverse reactions occur. Results All 60 patients, 34 underwent radical gastrectomy, 23 underwent palliative gastrectomy, 3 patients of pancreaticoduodenal resection, average cleaning the lymph nodes （23.4±1.4）; Except 1 case of postoperative death from pancreatic leakage, all of the patients after surgical treatment of the clinical symptoms were improved obviously; The study of the communist party of postoperative adverse reactions observed in 6 patients （10.0%）, 3 patients were treated by incision infection, 2 patients with pulmonary infection, 1 case of anastomotic stenosis, 1 case of patients with abdominal bleeding. Conclusion Anterograde method most of gastric resection in the treatment of gastric cancer has certain curative effect, to some extent, it can reduce the operation of pathological changes of stimuli, and conform to the surgical disease- free and without touching the basic principles of gastric cancer radical operation, it has clinical application value.
