三镜联合治疗肝外胆管结石的临床效果

Clinical effect of three-endoscopic combination in the treatment of extrahepatic bile duct stone

目的探讨三镜联合治疗肝外胆管结石的临床效果。方法选取本院2013年1月~2014年1月收治的80例肝外胆管结石患者作为研究对象,根据手术方法的不同将其分为治疗组与对照组,各40例。对照组采用常规手术治疗,治疗组测采用三镜（腹腔镜、十二指肠镜、胆道镜）联合治疗,比较两组的治疗效果。结果两组术后均未出现死亡病例。治疗组的术中失血量显著少于对照组,胃肠蠕动恢复时间、下床活动时间显著短于对照组,差异有统计学意义（P〈0.05）。治疗组的有效率显著高于对照组,差异有统计学意义（P〈0.05）。两组术后的主要并发症发生率比较,差异无统计学意义（P〉0.05）。结论三镜联合治疗肝外胆管结石效果显著,创伤小,出血量少,术后恢复快,值得临床推广应用。

Objective To explore the clinical effect of three-endoscopic combination in the treatment of extrahepatic bile duct stone. Methods 80 patients with extrahepatic bile duct stone in our hospital from January 2013 to January2014 were selected and divided into the treatment group and the control group according to the different operation methods,40 cases in each group.The control group was treated with conventional operation treatment,and the treatment group rwas given the treatment of three-endoscopic combination（laparoscope,duodenoscope and choledochoscope）.The treatment effect in two groups was compared. Results There was no death case in two groups after operation.The intraoperative blood loss in the treatment group was less than that in the control group,with significant difference（P〈0.05）.The recovery time of gastrointestinal motility and getting out-of-bed time in the treatment group was shorter than that in the control group,with significant difference（P〈0.05）.The total effective rate of the treatment group was higher than that of the control group,with significant difference（P〈 0.05）.There was no significant difference for incidence rate of complication between two groups after operation（P〉0.05）. Conclusion The effect of three-endoscopic combination in the treatment of extrahepatic bile duct stone is significant,trauma is small,the amount of bleeding is small,postoperative recovery is fast,it is worthy of clinical promotion and application.
